人工股骨头置换术治疗老年股骨粗隆间骨折的临床效果分析

Clinical Effect Analysis of Artificial Femoral Head Replacement in the Treatment of Elderly Femoral Intertrochanteric Fractures

目的评估人工股骨头置换术应用于老年股骨粗隆间骨折患者的有效性。方法选取2021年10月—2023年2月北京王府中西医结合医院收治的64例老年股骨粗隆间骨折患者,选取随机数表法,分为对照组、观察组,每组32例,对照组为股骨近端防旋髓内钉内固定方案,观察组为人工股骨头置换术方案,对比两组患者在手术相关指标、疼痛情况、髋关节功能以及并发症情况方面。结果观察组患者的术中出血量为(223.18±26.39)mL,明显高于对照组的(183.27±31.45)mL,差异有统计学意义(t=5.499,P<0.05);观察组患者的手术时间明显长于对照组,差异有统计学意义(P<0.05),术后恢复及住院时间明显短于对照组,差异有统计学意义(P<0.05);观察组患者术后疼痛(VAS)得分和髋关节功能(Harris)得分均明显高于对照组,差异有统计学意义(P<0.05);两组患者术后常见并发症的发生率均较少,差异无统计学意义(P>0.05)。结论老年股骨粗隆间骨折患者经人工股骨头置换术的科学救治后,术后恢复及住院时间较短,髋关节功能得到明显改善,且安全性较佳,临床效果显著。

Objective To evaluate the effectiveness of artificial femoral head replacement in elderly patients with intertrochanteric fractures.Methods From October 2021 to February 2023,64 elderly patients with intertrochanteric fracture of femur admitted to Beijing Wangfu Hospital of Integrated Traditional Chinese and Western Medicine were selected and randomly divided into control group and observation group,with 32 cases in each group.The control group was treated with proximal femoral anti-rotation intramedullary nail internal fixation,while the observation group was treated with artificial femoral head replacement.The operation-related indexes,pain,hip joint function and complications were compared between the two groups.Results The intraoperative blood loss of the observation group was(223.18±26.39)mL,which was significantly higher than that of the control group(183.27±31.45)mL,and the difference was statistically significant(t=5.499,P<0.05).The operation time of the observation group was significantly longer than that of the control group,and the difference was statistically significant(P<0.05).The postoperative recovery and hospitalization time were significantly shorter than those of the control group,and the difference was statistically significant(P<0.05).The postoperative pain(VAS)score and hip function(Harris)score of the observation group were significantly higher than those of the control group,and the differences were statistically significant(P<0.05).The incidence of common postoperative complications in the two groups was less,and the difference was not statistically significant(P>0.05).Conclusion After the scientific treatment of artificial femoral head replacement in elderly patients with intertrochanteric fractures,the postoperative recovery and hospitalization time are shorter,the hip joint function is significantly improved,and the safety is better,and the clinical effect is remarkable.
