微创拔牙器械配合高速涡轮牙钻拔牙法在下颌低位阻生智齿治疗中的效果评价

Evaluation on the efficacy of minimally invasive tooth extraction instrument combined with high-speed turbine dental drill in the treatment of lower impacted mandibular wisdom teeth

目的评价下颌低位阻生智齿患者应用微创拔牙器械配合高速涡轮牙钻拔牙法的疗效。方法选择山东国欣颐养集团枣庄中心医院2021年6月至2022年6月就诊的下颌低位阻生智齿患者76例,根据随机数表法分为两组,每组各38例,对照组采用传统拔牙术治疗,观察组施行微创拔牙器械配合高速涡轮牙钻拔牙法治疗,将两组下颌低位阻生智齿患者的手术时长、出血量、术后断根率、牙龈撕裂率、牙窝完整率、张口受限程度、生活质量、并发症发生率进行比较。结果观察组手术时长短于对照组,出血量小于对照组,差异有统计学意义(P<0.05)。观察组术后断根率、牙龈撕裂率小于对照组,牙窝完整率大于对照组,差异有统计学意义(P<0.05)。观察组张口受限率优于对照组,差异有统计学意义(P<0.05)。观察组生活质量综合评定问卷心理功能、物质生活状态、社会功能以及躯体功能评分大于对照组,差异有统计学意义(P<0.05)。观察组并发症(创面感染、出血肿胀、干槽症)总发生率小于对照组,差异有统计学意义(P<0.05)。结论下颌低位阻生智齿患者采用微创拔牙器械配合高速涡轮牙钻拔牙法的效果突出,不仅可以降低并发症发生率,还可改善患者手术情况,提高患者生存质量,值得推广。

Objective To evaluate the therapeutic efficacy of minimally invasive tooth extraction instrument combined with high-speed turbine dental drill on patients with lower impacted mandibular wisdom teeth.Methods A total of 76 cases with lower impacted mandibular wisdom teeth admitted to the Central Hospital of Zaozhuang City in Shandong Guoxin Yiyang Group from June 2021 to June 2022 were selected,and they were divided into the control group(n=38)and the observation group(n=38)according to the random number table method.The control group was treated with conventional tooth extraction,while the observation group was treated with minimally invasive tooth extraction instrument combined with high-speed turbine dental drill.The operation duration,hemorrhage volume,postoperative root fracture rate,gingival tearing rate,dental socket integrity rate,mouth opening restriction,quality of life(QoL)and incidence of complications of patients with lower impacted mandibular wisdom teeth of the two groups were compared.Results The operation duration of the observation group was shorter than that of the control group,and the blood loss of the observation group was lower than that of the control group,with statistically significant differences(P<0.05).The rates of root fracture and gingival tearing in the observation group were lower than those in the control group,and the rate of dental socket integrity in the observation group was higher than that in the control group,with statistically significant differences(P<0.05).The mouth opening restriction rate of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).The score of the psychological function,material life state,social function and physical function of generic quality of life inventory-74 in the observation group was higher than that in the control group,with statistically significant differences(P<0.05).The total incidence of complications(wound infection,hemorrhage and swelling and dry socket)in the observation group was lower than that in the control group,with statistically significant difference(P<0.05).Conclusion The efficacy of minimally invasive tooth extraction instrument combined with high-speed turbine dental extraction is superior in patients with lower impacted mandibular wisdom teeth,which can not only reduce the incidence of complications,but also improve the surgical situation and QoL of patients.Therefore,it is worthy of promotion.
