强化排尿功能训练在根治性膀胱全切回肠原位新膀胱术后患者护理中的应用

Application of Intensive Urination Training in Nursing Care of Patients after Radical Cystectomy and Ileal Orthotopic Neobladder Surgery

目的探讨强化排尿功能训练在根治性膀胱全切回肠原位新膀胱术后患者护理中的应用效果。方法研究对象为2018年12月至2019年12月我院收治的90例膀胱癌患者,按就诊顺序编号,均分为两组,每组45例。奇数编号为对照组,偶数编号为观察组。全部患者均接收根治性膀胱全切+全切回肠原位新膀胱术治疗,将常规护理干预措施应用于对照组,观察组患者在此基础上采取术后排尿功能强化训练。比较两组患者第一次排尿时间、排尿状态及尿潴留发生情况。比较两组患者护理满意度及简明健康状况量表(SF-36)评分。结果将两组患者第一次排尿时间进行对比,观察组明显短于对照组(P<0.05)。将两组患者排尿顺利情况进行比较,观察组明显优于对照组(P<0.05)。将两组患者尿潴留发生率进行对比,观察组明显低于对照组(P<0.05)。将两组患者护理满意度及生活质量评分进行对比,观察组明显高于对照组(P<0.05)。结论根治性膀胱全切回肠原位新膀胱术后患者采取排尿功能强化训练能够有效缩短患者第一次排尿时间,改善患者症状情况,对于降低尿潴留发生风险及提高其生活质量具有积极作用。

Objective To explore the application effect of intensive urination training in nursing care of patients after radical cystectomy and ileal neobladder surgery.Methods Ninety patients with bladder cancer in our hospital from December 2018 to December 2019 were divided into two groups,45 cases in each group.Odd number was control group,even number was observation group.All patients received radical cystectomy and ileal neobladder in situ.The routine nursing intervention was applied to the control group,and the observation group was given intensive training of postoperative urination function on this basis.The first voiding time,voiding state and urinary retention were compared between the two groups.The nursing satisfaction and 36-item Short-form Health Survey(SF-36)scores of the two groups were compared.Results The first urination time of the two groups was compared,and the observation group was significantly shorter than the control group(P<0.05).Compared the two groups of patients with smooth urination,the observation group was significantly better than the control group(P<0.05).The incidence of urinary retention in the two groups was compared,and the observation group was significantly lower than that in the control group(P<0.05).The two groups of patients with nursing satisfaction and quality of life scores were compared,the observation group was significantly higher than the control group(P<0.05).Conclusion Intensive micturition and functional orthotopic neobladder can effectively reduce the first voiding time and improve the symptoms of patients.It has a positive effect on reducing the risk of urinary retention and improving the quality of life.
