术前心理状态对患者腹腔镜胰十二指肠切除术后康复的影响

Effect of Preoperative Psychological State on the Rehabilitation of Patients After Laparoscopic Pancreatoduodenectomy

目的评估腹腔镜胰十二指肠切除术患者术前心理状态,探究其对术后康复的影响。方法收集2020年1月至2022年7月在河南省人民医院肝胆外科接受腹腔镜胰十二指肠切除术治疗的132例患者,应用焦虑自评量表(SAS)和抑郁自评量表(SDS)评估术前心理状态,根据SAS及SDS评分将患者分为负面情绪组(41例)和对照组(91例),并对两组患者围手术期情况进行分析。结果两组年龄、体重指数、手术总时间、淋巴结清除数量、胰肠吻合时间、术中出血量、术中输血、性别、糖尿病、黄疸、病理诊断差异无统计学意义(P>0.05)。负面情绪组术后引流管拔管时间、术后住院时间、术后并发症Clavien-Dindo分级、术后肺部感染、二次手术以及胃瘫比率高于对照组(P<0.05)。结论术前负面情绪可增加患者并发症的发生,延缓腹腔镜胰十二指肠切除术患者术后恢复。

Objective To evaluate the psychological state of patients undergoing laparoscopic pancreaticoduodenectomy and explore its influence on postoperative rehabilitation.Methods A total of 132 patients hospitalized in the Department of Hepatobiliary and Pancreatic Surgery of Henan Provincial People’s Hospital from January 2020 to July 2022 who received laparoscopic pancreaticoduodenectomy were selected and their psychological status was evaluated by self-rating anxiety scale(SAS)and self-rating depression scale(SDS)before surgery.According to SAS and SDS scores,the patients were divided into negative emotion group(41 cases)and control group(91 cases),and the perioperative conditions of the two groups were analyzed.Results There were no statistically significant differences between the two groups in age,body mass index,total operation time,number of lymph node clearance,time of pancreaticoenteric kissing,intraoperative blood loss,intraoperative blood transfusion,gender,diabetes,jaundice and pathological diagnosis(P>0.05).The time of drainage tube extraction,postoperative hospital stay,Clavien-Dindo grade of postoperative complications,postoperative pulmonary infection,secondary operation and gastroparesis rates in negative emotion group were higher than that in control group(P<0.05).Conclusion Preoperative negative emotion can increase the incidence of complications and delay postoperative recovery in patients with laparoscopic pancreatoduodenectomy.
