CT联合MRI对胫骨平台隐匿性骨折的诊断价值和符合率评价

Diagnostic Value and Coincidence Rate of CT Combined with MRI in Occult Fracture of Tibial Plateau

目的:对疑似胫骨平台隐匿性骨折92例患者,临床采用计算机体层成像(CT)、磁共振成像(MRI)检查诊断,统计联合诊断效能。方法:自2019年11月~2020年10月本院所收患者中选取92例疑似胫骨平台隐匿性骨折患者,均行MRI、CT检查,以病理结果为“金标准”,统计结果,分析不同影像学检查诊断结果。结果:92例疑似病例经病理检出阳性51例、阴性41例;CT检出阳性45例(真阳性42例),阴性47例(真阴性38例);MRI检出阳性46例(真阳性44例),阴性46例(真阴性39例);MRI+CT检出阳性51例(真阳性50例),阴性41例(真阴性40例)。MRI+CT检查诊断胫骨平台隐匿性骨折的符合率(97.83%)、灵敏度(98.04%)相对单独诊断均较高(P<0.05)。结论:MRI联合CT检查诊断胫骨平台隐匿性骨折符合率高,灵敏度高,可减少漏诊,可被临床治疗作为参考。

Objective:92 patients with suspected occult fracture of tibial plateau were diagnosed by CT and MRI.Methods:92 patients with suspected occult fracture of tibial plateau were selected from our hospital(from November 2019 to October 2020).All patients underwent MRI and CT examinations.The pathological results were regarded as the“gold standard”.The statistical results were analyzed and the diagnostic results of different imaging examinations were analyzed.Results:92 suspected cases were pathologically positive in 51 cases and negative in 41 cases;CT was positive in 45 cases(true positive in 42 cases)and negative in 47 cases(true negative in 38 cases);MRI was positive in 46 cases(true positive in 44 cases)and negative in 46 cases(true negative in 39 cases);MRI+CT was positive in 51 cases(true positive in 50 cases)and negative in 41 cases(true negative in 40 cases).The coincidence rate(97.83%)and sensitivity(98.04%)of MRI combined with CT in the diagnosis of occult fracture of tibial plateau were higher than those of single diagnosis(P<0.05).Conclusion:MRI combined with CT in the diagnosis of occult fracture of tibial plateau has high coincidence rate and high sensitivity,which can reduce missed diagnosis and can be used as a reference for clinical treatment.
