摘要 目的:探讨有创-无创呼吸机序贯机械通气联合肺表面活性剂治疗新生儿呼吸窘迫综合征患者的安全性和有效性。方法:选取2022年10月~2023年3月收治的80例呼吸窘迫综合征新生儿,随机分为对照组(n=40)和观察组(n=40),对照组新生儿用有创机械通气联合肺表面活性剂治疗,观察组新生儿用有创与无创呼吸机序贯疗法联合肺表面活性剂治疗。比较两组患者的器械通气时间、住院时间、血气指标(PaCO_(2)与PaO_(2))、有效率和并发症发生率。结果:尽管观察组和对照组的有效率无显著性差异,但与对照组相比,观察组可显著缩短新生儿器械通气时间和住院时间,观察组对新生儿血气指标的改善效果更好,而且观察组还可显著降低新生儿的并发症发生率。结论:有创-无创呼吸机序贯机械通气联合肺表面活性剂在新生儿呼吸窘迫综合征患者中的应用是安全有效的。 Objective:To investigate the safety and effectiveness of invasive and noninvasive sequential mechanical ventilation combined with pulmonary surfactant in the treatment of neonatal respiratory distress syndrome.Methods:This study was carried out from October 2022 to March 2023,80 neonates with respiratory distress syndrome randomly divided into the control group(n=40)and the observation group(n=40).The neonates in the control group were treated with invasive mechanical ventilation combined with pulmonary surfactant,while the neonates in the observation group were treated with invasive and noninvasive sequential mechanical ventilation combined with pulmonary surfactant.The ventilation time,hospitalization time,blood gas indexes(PaCO_(2) and PaO_(2)),effective rate and complication rate of the two groups were compared.Results:There is no significant difference in the effective rate between the observation group and the control group,however,compared with the control group,the observation group can significantly shorten the neonatal ventilation time and hospitalization time.The observation group also has a better effect on the improvement of neonatal blood gas indexes,and the observation group can significantly reduce the neonatal complication rate.Conclusion:Invasive and noninvasive sequential mechanical ventilation combined with pulmonary surfactant is safe and effective in the treatment of neonatal respiratory distress syndrome.
