对比研究根治性外放射治疗和根治性前列腺切除术治疗局限性高危前列腺癌的临床效果

Objective to Compare the Clinical Effect of Radical External Radiotherapy and Radical Prostatectomy in the Treatment of Localized High-Risk Prostate Cancer

目的:研究根治性外放射治疗和根治性前列腺切除术治疗局限性高危前列腺癌的临床效果。方法:回顾性分析2015年7月~2019年11月本院收治的88例诊断为局限性高危前列腺癌(局限性高危前列腺癌入院标准:前列腺特异性抗原≥20ng/mL或cT3以上)患者临床资料。根据患者采用的不同治疗方法分为对照组(根治性前列腺切除术治疗,48例)与观察组(采用根治性外放射治疗,40例)。治疗后随访1年,比较两组生化复发率、无远处转移生存时间、无生化复发生存时间、总生存时间。结果:观察组生化复发率低于对照组,差异有统计学意义(P<0.05)。观察组无生化复发患者生存时间长于对照组,差异有统计学意义(P<0.05);观察组与对照组患者的无远处转移生存时间、总生存时间比较,差异无统计学意义(P>0.05)。结论:在局限性高危前列腺癌治疗中采用根治性外放射治疗的效果优于根治性前列腺切除术,可显著降低患者生化复发率,延长无生化复发患者生存时间。

Objective:To study the clinical effect of radical external radiotherapy and radical prostatectomy in the treatment of localized high-risk prostate cancer.Methods:The clinical data of 88 patients diagnosed as localized high risk prostate cancer(admission standard of localized high-risk prostate cancer:prostate specific antigen(PAS)≥20ng/mL or cT3 or above)were analyzed retrospectively.According to the different treatment methods,the patients were divided into reference group(radical prostatectomy,48 cases)and research group(40 cases with radical external radiotherapy).The biochemical recurrence rate,distant metastasis free survival time,biochemical recurrence free survival time and overall survival time were compared between the two groups.Results:The biochemical recurrence rate of the study group was lower than that of the control group(P<0.05).The survival time of patients without biochemical recurrence in the study group was longer than that in the control group,and the difference was statistically significant(P<0.05);There was no significant difference in distant metastasis free survival time and overall survival time between the study group and the reference group(P>0.05).Conclusion:In the treatment of localized high-risk prostate cancer,the effect of radical external radiotherapy is better than radical prostatectomy,which can significantly reduce the biochemical recurrence rate and prolong the survival time of patients without biochemical recurrence.
