中医定向透药疗法配合针灸治疗腰椎间盘突出症的临床疗效探讨

Discussion on clinical efficacy of traditional Chinese medicine directional penetration therapy combined with acupuncture in the treatment of lumbar disc herniation

目的 探究与分析中医定向透药疗法配合针灸治疗腰椎间盘突出症(LDH)的临床疗效。方法 90例LDH患者,采取随机数字表法分为对照组与观察组,各45例。两组均给予消炎镇痛、消除神经根水肿等基础治疗,对照组给予中医定向透药疗法治疗,观察组在对照组基础上加用针灸治疗。对比两组临床疗效及治疗前后白细胞介素-1(IL-1)、C反应蛋白(CRP)、视觉模拟评分法(VAS)评分、日本骨科协会评估治疗(JOA)评分。结果 观察组临床总有效率91.11%高于对照组的73.33%,差异有统计学意义(P<0.05)。治疗后,两组IL-1及CRP水平均低于本组治疗前,且观察组IL-1及CRP水平分别为(0.99±0.10)μg/L、(3.34±0.43)mg/L,低于对照组的(1.85±0.07)μg/L、(5.61±0.77)mg/L,差异有统计学意义(P<0.05)。治疗后,两组VAS评分均低于本组治疗前,JOA评分均高于本组治疗前,且观察组VAS评分(2.61±0.40)分低于对照组的(4.86±0.77)分,JOA评分(25.23±2.76)分高于对照组的(20.98±4.21)分,差异有统计学意义(P<0.05)。结论 中医定向透药疗法配合针灸治疗LDH可获得较好的临床疗效,调节IL-1及CRP水平,降低疼痛感,改善腰椎活动功能。

Objective To investigate and analyze the clinical efficacy of traditional Chinese medicine directional penetration therapy combined with acupuncture in the treatment of lumbar disc herniation(LDH).Methods A total of 90 patients with LDH were randomly divided into control group and observation group,with 45 cases in each group.Both groups were given anti-inflammatory,analgesic,elimination of nerve root edema and other basic treatment.On this basis,the control group was given traditional Chinese medicine directional penetration therapy,and the observation group was given acupuncture treatment on the basis of the control group.Both groups were compared in terms of clinical efficacy,interleukin-1(IL-1),C-reactive protein(CRP),visual analogue scale(VAS)score and Japanese Orthopaedic Association(JOA)score before and after treatment.Results The total clinical effective rate of 91.11%in the observation group was higher than that of 73.33%in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of IL-1 and CRP in both groups were lower than those before treatment in this group;the levels of IL-1 and CRP were(0.99±0.10)μg/L and(3.34±0.43)mg/L in the observation group,which were lower than those of(1.85±0.07)μg/L and(5.61±0.77)mg/L in the control group;the differences were statistically significant(P<0.05).After treatment,VAS score in both groups was lower than that before treatment in this group,and JOA score was higher than that before treatment in this group;VAS score of(2.61±0.40)points in the observation group was lower than that of(4.86±0.77)points in the control group;JOA score of(25.23±2.76)points in the observation group was higher than that of(20.98±4.21)points in the control group;the differences were statistically significant(P<0.05).Conclusion Traditional Chinese medicine directional penetration therapy with acupuncture can achieve better clinical efficacy in treating LDH,regulating IL-1 and CRP levels,reducing pain and improving lumbar mobility.
