正畸牵引联合牙冠延长术治疗上前牙龈缺损的效果及对患者牙周健康的影响研究

The effect of orthodontic traction combined with crown lengthening in the treatment of upper anterior gingival defect and its influence on patients'periodontal health

目的探讨正畸牵引联合牙冠延长术治疗上前牙龈缺损的效果及对患者牙周健康的影响。方法此研究选取2022年1月-2023年1月广西壮族自治区中医药研究院收治的76例上前牙龈缺损患者,按随机数字表法分组,手术组接受常规牙冠延长术治疗(38例),联合组在手术组治疗基础上加用正畸牵引(38例),比较两组牙周功能改善情况、修复疗效及炎症反应,并调查患者对修复的满意程度。结果联合组治疗后龈沟出血指数、牙周袋探诊深度、菌落指数及牙龈指数低于手术组,联合组修复总有效率显著高于手术组;联合组治疗后龈沟液内白细胞介素-6(IL-6)及肿瘤细胞因子(TNF-α)含量明显低于手术组;联合组对修复的外形美观度、固位功能及舒适度方面满意度评分均高于手术组,差异有统计学意义(P<0.05)。结论正畸牵引联合牙冠延长术在临床上前牙龈缺损患者中的治疗可获得良好疗效,并能改善其牙周功能。

Objective To explore the effect of orthodontic traction combined with crown lengthening in the treatment of upper anterior gingival defect and its influence on patients'periodontal health.Methods In this study,76 patients with upper anterior gingival defect in our hospital from January 2022 to January 2023 were randomly divided into two groups.The operation group received conventional crown lengthening surgery(38 cases),and the combined group received orthodontic traction(38 cases)on the basis of the treatment of the operation group.The improvement of periodontal function,repair effect and inflammatory reaction of the two groups were compared,and the satisfaction of patients with the repair was investigated.Results After treatment,the gingival sulcus bleeding index,periodontal pocket probing depth,colony index and gingival index of the combined group were lower than those of the operation group,and the total effective rate of the combined group was significantly higher than that of the operation group;the levels of Interleukin-6(IL-6)and tumor cell factor(TNF-α)in gingival crevicular fluid in combination group after treatment was significantly lower than that of the operation group;the satisfaction scores of cosmetic appearance,retention function and comfort in the combined group were higher than those in the operation group,with statistically significant difference(P<0.05).Conclusion Orthodontic traction combined with crown lengthening can achieve good results in the treatment of anterior gingival defect and improve its periodontal function.
