男性不育症精子DNA完整结构损伤的中医药治疗研究进展

Research progress of traditional Chinese medicine in the treatment of sperm DNA integrity structure damage in male infertility

近年来,随着男性不育症相关致病因素更深入的研究,男性精子DNA碎片指数成为新的评估男性生育力和预测临床结局的重要指标之一,精子DNA完整结构的异常直接影响胚胎的质量和临床结局,严重者可导致女性反复着床失败和流产。中医学对类似男性不育症的认识和研究早有记载,以“浊邪”为病机辨治精子DNA损伤越来越被学者们所认可,认为“浊”与男性精子DNA完整结构的损伤有直接的关联性,并强调湿、热、瘀以及滞等统称为浊邪,对精子DNA完整结构有破坏作用,认为浊邪不单是致病因素也是病理产物,并强调化浊法对精子DNA结构损伤的重要修复作用。笔者根据查阅文献并结合生殖男科门诊用药经验归纳总结了中医药对不育症伴精子DNA损伤患者的治疗,为临床采用中医药对不育症伴精子DNA损伤的诊疗提出新思路。

In recent years,with the in-depth study of the pathogenic factors related to male infertility,the male sperm DNA fragmentation index has become one of the new important indicators to evaluate male fertility and predict clinical outcomes.The abnormal integrity of sperm DNA structure directly affects the quality of embryos and clinical outcomes,and in severe cases,it can lead to repeated implantation failure and abortion.The understanding and research of similar male infertility in traditional Chinese medicine have been recorded for a long time,and the identification and treatment of sperm DNA damage with"turbiditism"as the pathogenesis has been increasingly recognized by scholars.It is believed that"turbiditism"has a direct correlation with the damage of sperm DNA integrity structure,and emphasizes that dampness,heat,blood stasis,stag-nation and so on are collectively called turbiditism,which can damage the integrity of sperm DNA structure.It is believed that turbidization is not only a pathogenic factor but also a pathological product.It is emphasized that turbidization plays an important role in the repair of sperm DNA structural damage.Based on the literature review and the experience of drug use in reproductive andrology clinic,the author summarizes the treatment of infertility patients with sperm DNA damage with traditional Chinese medicine,and puts forward new ideas for the clinical use of traditional Chinese medicine in the diagnosis and treatment of infertility with sperm DNA damage.
