慢性阻塞性肺病患者血清CCL17的变化及其与肺功能的关系

Changes of Serum CCL17 in Patients with Chronic Obstructive Pulmonary Disease and Its Relationship with Lung Function

摘要 为探究CCL17在COPD患者中的变化及其与肺功能和疾病严重程度的关系,选取2021年1月至2022年5月间就诊的62名COPD患者和50名健康对照组作为研究对象.COPD患者中包括稳定期COPD组38例和AECOPD组24例.采用酶联免疫吸附实验测定各组血清CCL17的质量浓度;采用Spearman法分析CCL17与临床和实验室指标间的相关性.结果显示:SCOPD、AECOPD和对照组间年龄、性别构成、BMI及吸烟与否等资料间均无统计学意义(P>0.05).AECOPD患者具有更低水平的FEV1/FVC(%)和FEV1%,具有更高水平的PLT、CRP及IL⁃4,均显著高于SCOPD和对照组(P<0.05);以上指标在SCOPD与对照组间同样有统计学意义(P<0.05).AECOPD患者血清CCL17显著高于SCOPD和对照组(P<0.01),且SCOPD患者同样高于对照组(P<0.01).GOLD 3⁃4级的患者血清CCL17水平显著高于GOLD 1⁃2级的患者(P<0.05).CCL17与FEV1/FVC(%)和FEV1%呈显著负相关性,与CRP、IL⁃4及PLT呈显著正相关性.ROC曲线显示血清CCL17区分COPD与健康对照时AUC高达0.83,敏感性为72.73%,特异度为90.00%.综上所述,异常上调的CCL17与COPD疾病进展密切相关,或可成为COPD疾病评估的生物标志物. To investigate the change of CCL17 in COPD patients and its relationship with pulmonary function and disease severity.Sixty⁃two COPD patients and fifty healthy controls who were seen between January 2021 and May 2022 were selected for the study.Thirty⁃eight cases with stable COPD and 24 cases with AECOPD were included.Enzyme⁃linked immunosorbent assay was performed to determine the concentration of serum CCL17 in each group.The correlations between CCL17 and clinical and laboratory indices were analyzed by Spearman's method.The results show that there was no statistical difference between SCOPD,AECOPD and control groups in terms of age,gender composition,BMI and smoking(P>0.05).AECOPD patients had lower levels of FEV1/FVC(%)and FEV1%,and higher levels of PLT,CRP and IL⁃4 than SCOPD and control groups(P<0.05).Serum CCL17 was significantly higher in AECOPD patients than in SCOPD and controls(P<0.01).Serum CCL17 levels were significantly higher in patients with GOLD grade 3⁃4 than in patients with GOLD grade 1⁃2(P<0.05).CCL17 showed significant negative correlations with FEV1/FVC(%)and FEV1%and significant positive correlations with CRP,IL⁃4 and PLT.ROC curve showed a high AUC of 0.83,with a sensitivity of 72.73%and a specificity of 90.00%when using serum CCL17 to differentiate COPD from healthy controls.In conclusion,abnormal upregulation of CCL17 was closely associated with COPD disease progression and may be a biomarker for COPD disease assessment.
