沙格列汀结合预混胰岛素治疗2型糖尿病患者的临床效果

Clinical effect of saxagliptin combined with premixed insulin in the treatment of patients with type 2 diabetes mellitus

目的分析沙格列汀结合预混胰岛素治疗2型糖尿病患者的临床效果。方法选择2019年6月至2021年6月于我院诊治的84例2型糖尿病患者作为研究对象,按电脑盲选法将其分为对照组和研究组,每组42例。对照组采用预混胰岛素治疗,研究组采用沙格列汀结合预混胰岛素治疗。比较两组的治疗效果。结果治疗后,研究组的血糖波动系数(BGFC)、平均血糖(MBG)、平均血糖波动幅度(MAGE)及日间血糖平均绝对差(MODD)低于对照组(P<0.05)。治疗后,研究组的白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)水平及核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)、Caspase-1蛋白在外周血单核细胞中的表达低于对照组(P<0.05)。治疗后,研究组的丙二醛(MDA)、脂质过氧化氢(LHP)水平低于对照组,总抗氧化能力(T-AOC)、谷胱甘肽过氧化物酶(GSH-Px)水平高于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论沙格列汀结合预混胰岛素治疗2型糖尿病的效果显著,可减少患者的血糖波动,减轻机体炎症反应及氧化应激反应,且用药安全性高,值得临床进一步应用。

Objective To analyze the clinical effect of saxagliptin combined with premixed insulin in the treatment of patients with type 2 diabetes mellitus.Methods A total of 84 patients with type 2 diabetes mellitus who were diagnosed and treated in our hospital from June 2019 to June 2021 were selected as the research objects.According to the computer blind selection method,the patients were divided into control group and study group,with 42 cases in each group.The control group was treated with premixed insulin,and the study group was treated with saxagliptin combined with premixed insulin.The therapeutic effects of the two groups were compared.Results After treatment,the blood glucose fluctuation coefficient(BGFC),mean blood glucose(MBG),mean amplitude of glycemic excursions(MAGE)and means of daily difference(MODD)in the study group were lower than those in the control group(P<0.05).After treatment,the levels of interleukin-6(IL-6),interleukin-1β(IL-1β)and the expression of nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)and Caspase-1 protein in peripheral blood mononuclear cells in the study group were lower than those in the control group(P<0.05).After treatment,the levels of malondialdehyde(MDA)and lipid hydrogen peroxide(LHP)in the study group were lower than those in the control group,the levels of total antioxidant capacity(T-AOC)and glutathione peroxidase(GSH-Px)were higher than those in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The saxagliptin combined with premixed insulin in the treatment of type 2 diabetes mellitus has a significant effect,it can reduce the blood glucose fluctuation of patients,reduce the body inflammatory response and oxidative stress response,and has high drug safety,which is worthy of further clinical application.
