达格列净对血糖控制良好2型糖尿病患者血糖波动的影响

Effect of Dapagliflozin on Blood Glucose Fluctuation in Patients with Well controlled Type 2 Diabetes

目的研究达格列净对血糖控制良好的2型糖尿病患者血糖波动的影响。方法回顾性分析2020年1月—2022年10月泰州市第四人民医院住院的60例血糖控制好转后行72 h动态血糖监测的2型糖尿病患者的临床资料,按更换降糖方案不同分为对照组和观察组,各30例。其中予诺和锐30+二甲双胍治疗作为对照组,在对照组基础上加用达格列净治疗作为观察组。比较两组血糖平均值、葡萄糖标准差、变异系数、平均葡萄糖波动幅度、葡萄糖目标范围内时间及低血糖的发生率。结果观察组血糖平均值(7.09±0.96)mmol/L、葡萄糖标准差(1.75±0.74)mmol/L、变异系数(24.44±8.63)%及平均葡萄糖波动幅度(4.54±1.96)mmol/L均低于对照组,差异有统计学意义(t=3.279、3.707、2.314、2.199,P<0.05)。观察组葡萄糖目标范围内时间(88.93±11.08)%高于对照组,差异有统计学意义(t=-3.747,P<0.05)。观察组低血糖的发生率(16.7%)低于对照组,差异有统计学意义(P<0.05)。结论达格列净可减少血糖波动及低血糖发生率。

Objective To study the effect of dapagliflozin on blood glucose fluctuation in patients with type 2 diabetes with good blood glucose control.Methods Retrospective analysis was made on the clinical data of 60 patients with type 2 diabetes who were hospitalized in Taizhou Fourth People's Hospital from January 2020 to October 2022 and underwent 72 h dynamic blood glucose monitoring after their blood glucose control improved.They were divided into the control group and the observation group according to the change of glucose reduction scheme,with 30 cases in each group.Among them,the control group was treated with Novolab 30+Metformin,and the observation group was treated with daggliflozin in addition to the control group.Compared the average blood glucose values,glucose standard deviation,coefficient of variation,average glucose fluctuation amplitude,time within the glucose target range,and incidence of hypoglycemia between the two groups.Results The mean blood glucose(7.09±0.96)mmol/L,standard deviation of glucose(1.75±0.74)mmol/L,coefficient of variation(24.44±8.63)%and mean glucose fluctuation(4.54±1.96)mmol/L in observation group were lower than those in control group,the difference was statistically significant(t=3.279,3.707,2.314,2.199,P<0.05).The glucose target range time of observation group(88.93±11.08)%was higher than that of control group,and the difference was statistically significant(t=-3.747,P<0.05).The incidence of hypoglycemia in the observation group(16.7%)was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Dapagliflozin can reduce blood glucose fluctuations and the incidence of hypoglycemia.
