彩色多普勒超声在糖尿病患者下肢动脉病变诊断中的应用

Application of Color Doppler Ultrasound in Diagnosis of Lower Extremity Arterial Disease in Diabetic Patients

目的探讨糖尿病患者下肢动脉病变诊断中彩色多普勒超声的应用价值。方法回顾性选取2020年2月—2022年2月山东省临沂市兰山区人民医院糖尿病患者50例作为糖尿病组、健康体检人员50名作为健康组,均接受彩色多普勒超声。统计分析两组下肢动脉IMT、下肢动脉硬化病变发生情况、股动脉血流参数、腘动脉血流参数、下肢血管血液流变学。结果糖尿病组患者的股动脉、腘动脉、胫后动脉IMT均大于健康组,差异有统计学意义(P<0.05);股动脉、腘动脉、胫后动脉、足背动脉硬化斑块发生率均高于健康组,血管狭窄、血管闭塞发生率高于健康组,差异有统计学意义(P<0.05);腘动脉EDV、PSV、PI均低于健康组,RI高于健康组,差异有统计学意义(P<0.05)。两组患者的股动脉、腘动脉、胫后动脉、足背动脉的血流量、血管内径、频谱宽度均逐渐降低,差异有统计学意义(P<0.05);糖尿病组患者的股动脉、腘动脉、胫后动脉、足背动脉的血流量、血管内径均低于健康组,频谱宽度均高于健康组,差异有统计学意义(P<0.05)。结论糖尿病患者下肢动脉病变诊断中彩色多普勒超声的应用价值高。

Objective To investigate the application value of color Doppler ultrasound in the diagnosis of lower ex-tremity arterial disease in diabetic patients.Methods Fifty patients with diabetes in Lanshan District People´s Hospital,Linyi,Shandong Province from February 2020 to February 2022 were retrospectively selected as the diabetic group and fifty healthy physical examination personnel were selected as the healthy group.All of them received color Doppler ultrasound.Lower limb artery IMT,incidence of lower limb arteriosclerosis,femoral artery blood flow param-eters,popliteal artery blood flow parameters and lower limb blood rheology were statistically analyzed in the two groups.Results The IMT of femoral artery,popliteal artery and posterior tibial artery in diabetic group were higher than those in healthy group,and the difference was statistically significant(P<0.05).The incidence of atherosclerotic plaque in femoral artery,popliteal artery,posterior tibial artery and dorsal foot artery were higher than those in healthy group,the incidence of vascular stenosis and vascular occlusion was higher than that in healthy group,the difference was statistically significant(P<0.05).The popliteal artery EDV,PSV and PI were lower than those in the healthy group,while RI was higher than that in the healthy group,the difference was statistically significant(P<0.05).Blood flow,vessel diameter and spectrum width of femoral artery,popliteal artery,posterior tibial artery and dorsalis pedis in two groups were decreased gradually,and the differences were statistically significant(P<0.05).The blood flow and in-ner diameter of femoral artery,popliteal artery,posterior tibial artery and dorsal pedis in diabetic group were lower than those in healthy group,and the spectrum width was higher than that in healthy group,and the difference was sta-tistically significant(P<0.05).Conclusion The application value of color Doppler ultrasound in the diagnosis of lower extremity arterial disease in diabetic patients is high.
