基于肠内营养耐受评估的标准化流程管理对重症监护室机械通气患者胃肠功能及营养状况的影响

摘要 目的研究分析基于肠内营养耐受评估的标准化流程管理对重症监护室机械通气患者胃肠功能及营养状况的影响。方法选取2021年7月至2022年12月在我院重症监护室接受机械通气治疗的患者88例为研究对象,根据随机抽签法将患者分成两组,对照组44例、研究组44例。对照组实施常规护理,研究组实施基于肠内营养耐受评估的标准化流程管理,观察并比较两组患者胃肠功能、营养状况、免疫功能、生活质量差异。结果与对照组比较,研究组胃肠功能障碍发生率更低(P<0.05)。相比于对照组,研究组干预后血清白蛋白(ALB)、前白蛋白(PAB)、总蛋白(TP)、血红蛋白(Hb)水平更高(P<0.05)。与对照组相比,研究组干预后免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)水平更高(P<0.05)。相较于对照组,研究组干预后生活质量各指标评分更高(P<0.05)。结论重症监护室机械通气患者实施基于肠内营养耐受评估的标准化流程管理后,有助于减少患者胃肠功能障碍的发生,能改善患者营养状况及免疫功能,能提高患者生活质量。 Objective To study and analyze the impact of standardized process management based on the evaluation of enteral nutrition tolerance on gastrointestinal function and nutritional status of patients with mechanical ventilation in intensive care unit.Methods Eighty-eight patients who received mechanical ventilation treatment in the intensive care unit of our hospital from July 2021 to December 2022 were selected as the study subjects.The patients were divided into two groups according to the method of random drawing,44 in the control group and 44 in the study group.The control group implemented routine nursing,and the study group implemented standardized process management based on the evaluation of enteral nutrition tolerance.The gastrointestinal function,nutritional status,immune function and quality of life of patients in the two groups were observed,and then the observation results of the two groups were statistically compared.Results Compared with the control group,the incidence of gastrointestinal dysfunction in the study group was lower(P<0.05).Compared with the control group,the levels of ALB,PAB,TP and Hb in the study group were higher after intervention(P<0.05).Compared with the control group,the levels of IgA,IgG and IgM in the study group were higher after intervention(P<0.05).Compared with the control group,the scores of quality of life indicators in the study group were higher after intervention(P<0.05).Conclusion The implementation of standardized process management based on the evaluation of enteral nutrition tolerance in patients with mechanical ventilation in intensive care unit can help reduce the occurrence of gastrointestinal dysfunction,improve the nutritional status and immune function of patients,and improve the quality of life of patients,which is worthy of clinical reference and promotion.
