摘要
目的运用Meta分析系统评价普拉提运动对下腰痛(LBP)的干预效果,探索可能产生较大干预量的训练方案。方法运用Review Manager 5.4对纳入的14篇随机对照试验研究,758名受试者的结局指标进行整体效应分析,并设置5个调节变量进行亚组分析。结果1)普拉提运动对改善下腰痛呈大效应量,d=-1.21,95%CI为[-1.71,-0.71];2)调节变量检验发现下腰痛改善效果较少受训练周期、训练频率和样本量的影响,但受普拉提类型和单次时长的影响。结论1)普拉提运动能有效改善下腰痛,器械普拉提比垫上普拉提的干预效果更为突出;2)采用每周训练3次、单次时长为25~50min左右、共持续4~6周左右的普拉提运动方案改善下腰痛的干预效果更优;3)样本量对实验干预效果产生一定程度的影响,建议未来RCT研究重视样本量的科学估算。
Objective To evaluate the effectiveness of pilates exercise on low back pain(LBP)through a systematic review by Meta-analysis,and to explore the training program that may produce a large intervention volume.Methods Using Review Manager 5.4 to analyze the overall effect of outcome indicators of 758 subjects in 14 literatures of randomised controlled trials(RCTs),and set 5 regulatory variables for subgroup analysis.Results 1)There was a large effect of pilates exercise on LBP(d=-1.21,95%CI[-1.71,-0.71]).2)The adjustment variable test found that the improvement effect of LBP was less affected by training cycle,training frequency and sample size,but affected by pilates type and single time duration.Conclusion 1)Pilates exercise can effectively improve LBP,and the intervention effect of equipment Pilates is more prominent than that of mat pilates.2)Pilates exercise program with 3 times a week,a single time of about 25~50 minutes,and a total of about 4~6 weeks was more effective in improving LBP.3)The sample size has a certain impact on the effect of experimental intervention,and it is suggested that future RCTs should pay attention to the scientific estimation of sample size.
