首诊于眼科的脑出血致皮质盲1例

Cortical blindness caused by cerebral hemorrhage in ophthalmology: a case report

皮质盲(cortical blindness, CB)是由各种原因引起的外侧膝状体至大脑枕叶视皮质通路受损所致的急性双眼视力丧失。皮质盲最常见的病因是脑血管疾病,其中脑梗死较多,极少数为脑出血。皮质盲临床较为罕见,因其临床特殊性,常就诊于眼科,容易被漏诊误诊,其诊断取决于神经科体征及影像学检查,且治疗效果不佳,甚至可导致不可逆性完全失明。早发现、早诊断、早治疗是提高患者生存质量的关键。现将首诊于本院眼科的脑出血致皮质盲1例报道如下。

Cortical blindness(corticalblindness,CB)is an acute binocular vision loss caused by damage to the visual cortex from the lateral geniculate body to the occipital lobe caused by various causes.The most common cause of cortical blindness is cerebrovascular disease,in which there are more cerebral infarction and very few are cerebral hemorrhage.CB is rare and easy to be misdiagnosed and missed.Its diagnosis depends on the signs of neurology and imaging examination.CB have a worse prognosis and even leads to irreversible blindness.Early detection,early diagnosis and prompt treatment are the key to improve the quality of life of patients.A case of cortical blindness caused by cerebral hemorrhage in the ophthalmology department of our hospital is reported as follows.
