社区老年患者慢阻肺控制水平的影响因素分析

Influence Factors of the Disease Control Conditions for Elderly Community Patients with Chronic Obstructive Pulmonary Disease

目的研究社区老年患者慢性阻塞性肺疾病(COPD)控制水平的影响因素。方法使用多阶段分层抽样法,选择郑州交通医院2019年12月至2021年12月期间收治的84例慢阻肺患者进行研究,对所有患者疾病控制水平开展调查,对处于不同时期的患者进行调查,开展一般资料分析,了解患者控制水平较差的影响因素。结果84例患者中,急性组16例(19.05%),稳定组68例(80.95%)。两组患者年龄、家庭平均月收入、吸烟史、家族呼吸系统疾病史、激素使用、BMI指数比较,差异有统计学意义(P<0.05)。将上述因素带入Logistic回归方程计算,证实年龄>70岁、家庭平均月收入<5000、吸烟史、家族呼吸系统疾病史、激素使用、BMI指数,均会影响老年COPD患者控制水平。结论社区老年患者慢阻肺控制水平整体情况良好,诸多因素的出现均会导致患者控制水平不足,针对社区老年患者,需要加强自身营养,并主动学习慢阻肺相关知识,合理使用药物,减少急性发作。

Objective To analyze the influence factors of disease control conditions for elderly community patients with chronic obstructive pulmonary disease(COPD).Methods Through the multi-stage stratified method,a total of 84 COPD patients treated from December 2019 to December 2021 in Zhengzhou Jiaotong Hospital were selected.The disease control levels of all patients were investigated,and the patients in different periods were investigated.General data analysis was carried out to understand the influencing factors of poor patient control level.Results Among 84 patients,16(19.05%)were in the acute group and 68(80.95%)were in the stable group.There were statistically significant differences in age,family average monthly income,smoking history,family history of respiratory disease,hormone use,and BMI between the two groups(P<0.05).The above factors were put into the Logistic regression equation,and it was confirmed that age>70 years,average family monthly income<5000,smoking history,family history of respiratory disease,hormone use and BMI index would affect the control level of elderly COPD patients.Conclusions The overall control level of COPD in elderly patients in the community is good,but many factors will lead to insufficient control level.For elderly patients in the community,it is necessary to strengthen their own nutrition,actively learn COPD related knowledge,rational use of drugs,and reduce acute attacks.
