循证医学理论指导的规范化疼痛护理管理在行膝关节置换术患者中的应用

Application of standardized pain nursing management guided by evidence-based medicine in patients undergoing knee replacement

目的探讨循证医学理论指导的规范化疼痛护理管理在行膝关节置换术(KR)患者中的应用效果。方法选取126例于2021年1月至2022年9月在盐城市第一人民医院行KR治疗的患者进行研究,采用随机数字表法将其分入2组。63例对照组患者施以常规护理,63例观察组患者在常规护理的基础上施以循证医学理论指导的规范化疼痛护理管理。比较2组的视觉模拟评分法(VAS)评分、美国特种外科医院(HSS)评分、术后恢复情况和并发症发生情况。结果观察组患者在术后12 h、24 h、48 h和出院时的VAS评分均低于同期对照组(P均<0.05);术后5 d、出院时和出院3个月后,观察组的HSS评分均高于同期对照组(P均<0.05);观察组的首次下地时间早于对照组,膝关节屈曲≥90°用时和住院时间均短于对照组,并发症总发生率低于对照组(P均<0.05)。结论循证医学理论指导的规范化疼痛护理管理可促进行KR患者的术后恢复,缓解其疼痛程度,改善其膝关节功能,减少并发症发生。

Objective To explore the application effect of standardized pain nursing management guided by evidence-based medicine in patients undergoing knee replacement(KR).Methods A total of 126 patients who received KR at Yancheng No.1 Peoples Hospital from January 2021 to September 2022 were included in the study.They were divided into two groups through random number table method.The 63 patients in the control group received routine nursing,and the 63 patients in the observation group received standardized pain nursing management guided by evidence-based medicine on the basis of the routine nursing.The Visual Analogue Scale(VAS)scores,Hospital for Special Surgery(HSS)scores,postoperative recovery,and complications were compared between the two groups.Results The VAS scores of the observation group were lower than those of control group at 12,24,48 hrs after surgery and at discharge(all P<0.05).At 5 days after surgery,at discharge,and at 3 months after discharge,the HSS scores of the observation group were higher than those of the control group(all P<0.05).The time to first ambulation,the time taken for knee flexion≥90°,and the length of hospital stay in the observation group were shorter than those in the control group,and the total incidence of complications was lower than that in the control group(all P<0.05).Conclusion Standardized pain nursing management guided by evidence-based medicine can promote the postoperative recovery of patients undergoing KR,relieve their pain,improve their knee function,and reduce the complications.
