CysC和Hcy指标在2型糖尿病肾病患者中的异常情况及诊断效能分析

Analysis of the Abnormalities and Diagnostic Efficacy of CysC and Hcy Indicators in Patients with Type 2 Diabetic Nephropathy

目的分析CysC和Hcy指标在2型糖尿病肾病(type 2 diabetic nephropathy,T2DN)患者中的异常情况及诊断效能。方法回顾性分析2021年9月—2022年8月期间福建省三明市明溪县总医院收治的101例2型糖尿病(type 2 diabetes mellitus,T2DM)患者的临床资料,按照24 h尿微量白蛋白(U-mAlb)水平将其分为T2DN组(51例)与T2DM组(50例)。两组患者均接受CysC和Hcy检测,观察并比较两组CysC和Hcy指标的异常率、CysC和Hcy水平,以及CysC、Hcy单独及联合诊断T2DN的敏感度与特异度。分析CysC和Hcy表达与T2DN患者U-mAlb的相关性。结果T2DN组患者CysC异常率(72.55%)、Hcy异常率(68.63%)均较T2DM组(28.00%、18.00%)高,差异有统计学意义(χ^(2)=20.045、26.321,P<0.05)。T2DN组患者CysC和Hcy水平均较T2DM组高,差异有统计学意义(P<0.05)。CysC联合Hcy诊断T2DN的敏感度与特异度均高于CysC、Hcy单独诊断,差异有统计学意义(P<0.05)。CysC与Hcy二者间单独诊断T2DN的敏感度与特异度对比,差异无统计学意义(P>0.05)。Spearman相关性分析显示,T2DN组患者CysC和Hcy表达与U-mAlb呈正相关性(P<0.05)。结论T2DN患者中CysC和Hcy水平呈异常表达,联合诊断具有较高的临床应用效能。

Objective To analyze the abnormalities and diagnostic efficacy of CysC and Hcy indicators in patients with type 2 diabetic nephropathy(Type 2 diabetic nephropathy,T2DN).Methods The clinical data of 101 patients with type 2 diabetes(T2DM)admitted to the General Hospital of Mingxi County,Sanming,Fujian Province from September 2021 to August 2022 were retrospectively analyzed.They were divided into T2DN group(51 cases)and T2DM group(50 cases)according to the level of 24-hour urinary microalbumin(U-mAlb).Both groups of patients underwent CysC and Hcy tests,and the abnormal rates of CysC and Hcy indicators,levels of CysC and Hcy,as well as the sensitivity and specificity of CysC and Hcy alone and in combination for the diagnosis of T2DN were observed and compared.Analyzed the correlation between the expression of CysC and Hcy and U-mAlb in T2DN patients.Results The abnormal rates of CysC(72.55%)and Hcy(68.63%)in the T2DN group were higher than those in the T2DM group(28.00%,18.00%),and the difference was statistically significant(χ^(2)=20.045,26.321,P<0.05).The levels of CysC and Hcy in the T2DN group were higher than those in the T2DM group,and the difference was statistically significant(P<0.05).The sensitivity and specificity of CysC combined with Hcy in diagnosing T2DN were higher than those of CysC and Hcy alone,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the sensitivity and specificity of diagnosing T2DN between CysC and Hcy alone(P>0.05).Spearman correlation analysis showed that the expression of CysC and Hcy in T2DN patients was positively correlated with U-mAlb(P<0.05).Conclusion CysC and Hcy levels were abnormally expressed in patients with T2DN,and the combined diagnosis has a high clinical application efficacy.
