2型糖尿病期间应用胰岛素强化治疗的效果分析

Analysis of the Effect of Applying Insulin Intensive Therapy during Type 2 Diabetes Mellitus

目的观察2型糖尿病期间应用胰岛素强化治疗的效果分析。方法选取2019年9月—2020年9月上杭县医院收治的316例初次诊断为2型糖尿病患者,以随机数表法分为两组,各158例,其中对照组接受阿卡波糖治疗,研究组为胰岛素强化治疗。比较两组患者治疗前后血糖指标与胰岛素指标,统计治疗后的临床转归情况。结果治疗后两组血糖指标水平及胰岛功能HOMA-IR指标较治疗前明显下降,其中研究组下降程度更明显,而胰岛功能中的HOMA-β指标较治疗前有所提升,其中研究组提升程度更明显,差异有统计学意义(P<0.05)。研究组经治疗后的低血糖率、胰岛素治疗率更低,停药率与口服降糖药率更高,差异有统计学意义(P<0.05)。结论在2型糖尿病期间应用胰岛素强化治疗,可以显著的降低患者的血糖指标水平,改善胰岛功能,提高治疗后停药、口服降糖药率,改善临床转归情况,效果理想。

Objective To observe the analysis of the effect of applying insulin intensive therapy during type 2 diabetes mellitus.Methods A total of 316 patients with type 2 diabetes initially diagnosed in Shanghang County Hospital from September 2019 to September 2020 were selected and divided into two groups with 158 cases each by random number table method.The control group received acarbose treatment and the study group received insulin intensive treatment.Blood glucose index and insulin index before and after treatment were compared between the two groups,and the clinical outcome after treatment was statistically analyzed.Results After treatment,the level of blood glucose indexes and HOMA-IR indexes of pancreatic islet function in the two groups decreased significantly compared with those before treatment,among which the degree of decrease was more obvious in the study group,while HOMA-βindexes of pancreatic islet function improved compared with those before treatment,among which the degree of improvement was more obvious in the study group,and the difference was statistically significant(P<0.05).The rate of hypoglycemia and insulin therapy was lower and the rate of drug discontinuation with oral hypoglycemic agents was higher in the study group after treatment,and the difference was statistically significant(P<0.05).Conclusion The application of insulin intensive therapy during type 2 diabetes can significantly reduce the level of glycemic index of patients,improve the function of pancreatic islets,increase the rate of drug discontinuation and oral hypoglycemic drugs after treatment,and improve clinical outcomes,which is ideal.
