神经外科术后发生颅内感染的危险因素分析

Risk factors of intracranial infection after neurosurgery

目的:研究神经外科术后发生颅内感染(intracranial infection,ICI)的危险因素。方法:回顾性分析2019年6月至2022年6月我院收治的80例神经外科术后发生ICI患者的临床资料,另以年龄和性别为匹配因素按1∶2选取同期神经外科术后未感染者160例为对照组,采用条件Logistic回归模型分析ICI危险因素。结果:急诊手术(OR=2.255,95%CI:1.087~4.674)、手术时间长(OR=1.385,95%CI:1.016~1.888)、切口为污染切口(OR=3.037,95%CI:2.060~4.478)、手术室人数多(OR=1.293,95%CI:1.114~1.501)和术后引流时间长(OR=1.697,95%CI:1.012~2.847)是神经外科术后发生ICI的独立危险因素(P<0.05)。结论:神经外科术后发生ICI危险因素包括急诊手术、手术时间、污染切口、手术室人数以及引流时间,针对这些因素进行针对性护理干预有利于预防ICI发生。

Objective:To study the risk factors of intracranial infection(ICI)after neurosurgery.Methods:The clinical data of 80 patients with ICI after neurosurgery in our hospital from Jun 2019 to Jun 2022 were retrospectively analyzed,and 160 patients without infection after neurosurgery during the same period were enrolled as the control group,with 1∶2 matching based on age and gender.Conditional logistic regression model was performed on the statistically significant indicators to analyze the risk factors of ICI.Result:Emergency surgery(OR=2.255,95%CI:1.087-4.674),long duration of surgery(OR=1.385,95%CI:1.016-1.888),contaminated incisions(OR=3.037,95%CI:2.060-4.478),large number of people in operating room(OR=1.293,95%CI:1.114-1.501)and long postopera tive drainage time(OR=1.697,95%CI:1.012-2.847)were independent risk factors for ICI after neurosur gery(P<0.05).Conclusions:The risk factors of ICI after neurosurgery include emergency surgery,surgery duration,contaminated incisions,number of people in operating room and drainage duration.Targeted nursing intervention for these factors is beneficial to preventing the occurrence of ICI.
