摘要
目的 探讨细胞核因子-κB(NF-κB)/丝裂原活化蛋白激酶(MAPK)信号通路与甲状腺癌术后复发的相关性。方法 回顾性分析2017年5月—2019年5月收治的甲状腺癌103例,根据术后3年有无复发分为复发组26例与未复发组77例。比较2组临床资料及术前、术后出院时、术后3年外周血单核细胞(PBMC)中NF-κB、MAPK mRNA相对表达量及差值,分析术前二者与临床病理特征的相关性。分析术后出院时与术后3年NF-κB、MAPK mRNA相对表达量差值对术后复发的影响及其诊断价值。结果 复发组术前、术后出院时、术后3年PBMC中NF-κB、MAPK mRNA相对表达量高于未复发组(P<0.01)。PBMC中NF-κB、MAPK mRNA相对表达量与肿瘤直径(r=0.428、0.407)、临床分期(r=0.509、0.513)、淋巴结转移(r=0.471、0.448)呈正相关(P<0.01)。术后出院时与术后3年PBMC中NF-κB、MAPK mRNA相对表达量差值与甲状腺癌术后复发独立相关(P<0.05)。NF-κB、MAPK mRNA联合诊断甲状腺癌术后复发的曲线下面积大于单独诊断(P<0.05)。结论 NF-κB/MAPK信号通路与甲状腺癌患者术后复发有关,联合检测NF-κB、MAPK mRNA相对表达量可预测术后复发。
Objective To investigate the correlation of nuclear factor-κB(NF-κB)/mitogen-activated protein kinase(MAPK)signaling pathway with postoperative recurrence of thyroid cancer(TC).Methods In total,103 patients with TC treated from May 2017 to May 2019 in our hospital were retrospectively selected and divided into recurrence group(n=26)and non-recurrence group(n=77)according to the presence or absence of recurrence at 3 years after surgery.The clinical data and the difference in relative expressions of NF-κB and MAPK mRNA in peripheral blood mononuclear cells(PBMC)before surgery,at postoperative discharge and at 3 years after surgery were compared between the two groups,and the correlation of the two with the clinicopathological features before surgery was analyzed.The impact of relative expression of NF-κB and MAPK mRNA at discharge and at 3 years after surgery on postoperative recurrence and their diagnostic value were analyzed.Results The mRNA relative expression levels of NF-κB and MAPK in PBMC of recurrence group were higher than those of non-recurrence group before surgery,at discharge and at 3 years after surgery(P<0.01).The relative expressions of NF-κB and MAPK mRNA in PBMC were positively correlated with tumor diameter(r=0.428,0.407),clinical stage(r=0.509,0.513)and lymph node metastasis(r=0.471,0.448)(P<0.01).The difference in relative expression of NF-κB and MAPK mRNA in PBMC at discharge and at 3 years after surgery was independently correlated with postoperative recurrence of TC(P<0.05).The area under the curve of NF-κB and MAPK mRNA in combination in the diagnosis was larger than that of single diagnosis alone(P<0.05).Conclusion NF-κB/MAPK signaling pathway is associated with postoperative recurrence of TC patients,and combined detection of relative expression of NF-κB and MAPK mRNA can predict postoperative recurrence.
