自拟益气健脾解毒化瘀方联合艾曲泊帕治疗恶性肿瘤化疗相关性血小板减少症的临床效果

Clinical Effect of Self-prepared Yiqi Jianpi Jiedu Huayu Formula Combined with Eltrombopag in the Treatment of Chemotherapy-induced Thrombocytopenia of Malignant Tumors

目的 探讨自拟益气健脾解毒化瘀方联合艾曲泊帕治疗恶性肿瘤化疗相关性血小板减少症的临床效果。方法 选取2018年1月—2022年4月收治的78例恶性肿瘤化疗相关性血小板减少症作为研究对象,按照治疗方法分为观察组(自拟益气健脾解毒化瘀方联合艾曲泊帕治疗)和对照组(艾曲泊帕治疗),每组39例。观察2组治疗后的临床效果,并比较治疗前后症状积分、血清炎性因子[白细胞介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)]、细胞因子[血小板第4因子(PF4)、血小板生成素(TPO)、血管性血友病因子(vWF)]及凝血功能指标[血小板计数(PLT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)]。结果 治疗后,观察组临床总有效率为94.87%(37/39)高于对照组79.49%(31/39)(P<0.05)。治疗后,2组血清IL-10、TNF-α、PF4、TPO、vWF水平及各症状积分均低于治疗前,且观察组低于对照组(P<0.05)。治疗后,2组PLT高于治疗前,PT、APTT短于治疗前;且观察组PLT高于对照组,PT、APTT短于对照组(P<0.05)。结论 自拟益气健脾解毒化瘀方联合艾曲泊帕治疗恶性肿瘤化疗相关性血小板减少症,能够提高疗效,减轻临床症状及炎症反应,改善凝血功能。

Objective To investigate the clinical effect of Yiqi Jianpi Jiedu Huayu formula combined with Eltrombopag in the treatment of chemotherapy-induced thrombocytopenia of malignant tumors and their impact on serum levels of interleukin-10(IL-10)and tumor necrosis factor-α(TNF-α).Methods A total of 78 patients with chemotherapy-induced thrombocytopenia of malignant tumor treated from January 2018 to April 2022 were selected as the research subjects.According to the treatment methods,they were divided into the observation group(self-prepared Yiqi Jianpi Jiedu Huayu formula combined with Eltrombopag,n=39)and the control group(Eltrombopag treatment,n=39).The clinical effect of the two groups after treatment was observed.Symptom scores,serum inflammatory factors[interleukin-10(IL-10),tumor necrosis factor-α(TNF-α)],cytokines[platelet factor 4(PF4),thrombopoietin(TPO),von Willemarsten factor(vWF)]and coagulation function indexes[platelet count(PLT),prothrombin time(PT),activated partial thromboplastin time(APTT)]before and after treatment were compared.Results After treatment,the total effective rate of observation group was higher than that of control group[94.87%(37/39)vs.79.49%(31/39)](P<0.05).After treatment,serum levels of IL-10,TNF-α,PF4,TPO,vWF and symptom scores in the two groups were lower than those before treatment,and lower in the observation group than in the control group(P<0.05).After treatment,PLT was higher than that before treatment,while PT and APTT were shorter than those before treatment.The PLT of the observation group was higher than that of the control group,and the PT and APTT of the observation group were shorter than those of the control group(P<0.05).Conclusion The combination of self-prepared Yiqi Jianpi Jiedu Huayu formula and Eltrombopag in the treatment of chemotherapy-induced thrombocytopenia of malignant tumor can improve the curative effect,reduce clinical symptoms and inflammatory reaction,and improve coagulation function.
