丙戊酸镁联合喹硫平治疗双相情感障碍躁狂发作的临床效果分析

Clinical Effect Analysis of Magnesium Valproate Combined with Quetiapine in the Treatment of Manic Episodes of Bipolar Disorder

目的:探讨丙戊酸镁联合喹硫平治疗双相情感障碍躁狂发作的临床疗效。方法:选取2021年1月—2022年6月北京市朝阳区第三医院收治的双相情感障碍躁狂发作患者66例作为研究对象,按照随机数字表法分为对照组和试验组,各33例。对照组给予丙戊酸镁治疗,试验组给予丙戊酸镁联合喹硫平治疗。比较两组临床疗效、狂躁程度、躁狂症状相关指标、生存质量、不良反应发生率。结果:试验组治疗有效率高于对照组,差异有统计学意义(P=0.024);治疗后,试验组贝克拉范森躁狂自评量表评分低于对照组,差异有统计学意义(P=0.018);治疗后,试验组蒙哥马利抑郁量表、杨氏狂躁量表、双向情感障碍总体印象评定量表评分均低于对照组,差异有统计学意义(P<0.001);治疗后2、4个月,试验组生存质量评分均高于对照组,差异有统计学意义(P<0.05);试验组不良反应发生率低于对照组,差异有统计学意义(P=0.003)。结论:丙戊酸镁联合喹硫平治疗双相情感障碍躁狂发作的临床效果显著,有利于控制患者的病情,降低不良反应发生率和狂躁程度,生活质量显著提升。

Objective:To investigate the clinical efficacy of magnesium valproate combined with quetiapine in the treatment of manic episodes in bipolar disorder.Methods:From January 2021 to June 2022,sixty-six patients with manic episodes of bipolar disorder admitted to the Chaoyang District Third Hospital,Beijing were selected as the research objects.According to the random number table method,they were divided into the control group and the test group,with thirty-three cases in each group.The control group was treated with magnesium valproate,and the test group was treated with magnesium valproate combined with quetiapine.The clinical curative effect,degree of mania,index of manic symptoms,quality of life and incidence of adverse reactions were compared between the two groups.Results:The effective rate of treatment in the test group was higher than that in the control group,and the difference was statistically significant(P=0.024).After treatment,the scores of the Baker Lavansen mania self-rating scale in the test group were lower than those in the control group,and the difference was statistically significant(P=0.018).After treatment,the scores of the Montgomery depression scale,Young's mania scale,and bipolar impression rating scale in the test group were lower than those in the control group,and the difference was statistically significant(P<0.001).After 2 and 4 months of treatment,the quality of life scores of the test group were higher than those of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the test group was lower than that in the control group,and the difference was statistically significant(P=0.003).Conclusion:Magnesium valproate combined with quetiapine has a significant clinical effect in the treatment of manic episodes in bipolar disorder,which is beneficial to control the patient's condition,reduce the incidence of adverse reactions and mania,and significantly improve the quality of life.
