“四点一线”定位法应用于乳房肿块微创旋切术疗效分析

Analysis of Clinical effect of"four-point one-line"localization method in minimally invasive rotary resection of breast mass

目的 回顾性分析总结“四点一线”定位法在乳房肿块微创旋切术的临床效果及体会。方法 选择河南中医药大学第一附属医院乳腺外科112例行乳房良性肿块微创旋切术患者的临床资料,根据定位方式的不同,分为“四点一线”定位法组(55例)和传统定位法组(57例),观察组采用“四点一线”定位法,对照组采用传统定位法,两组患者的手术均由专业、熟练的同一手术团队操作,比较两组的调针次数、术中出血量、手术时长、术后疼痛评分、血肿发生情况。结果 观察组和对照组的调针次数别分为(1.31±1.63)次和(4.47±63.03)次,术中出血量分别为(14.82±14.01)mL和(38.25±19.74)mL,手术时长分别为(33.58±22.73)min和(44.65±22.47)min,术后疼痛评分分别为(1.02±1.05)分和(2.54±1.00)分,血肿发生直径分别为(1.36±5.48)mm和(24.65±19.60)mm, P值均小于0.05建议改为:观察组在调针次数、术中出血量、手术时长、术后疼痛评分、血肿发生方面均明显低于对照组(P<0.05);结论 “四点一线”定位法可显著提高定位准确性,缩减手术时长,减少术中出血量,降低术后疼痛程度,减少术后血肿发生率,具有较高的安全性及实用性。

Objective To retrospectively analyze and summarize the clinical effect and experience of the"four points and one line"positioning method in minimally invasive rotary excision of breast lumps.MethodsThe clinical data of 112 patients who underwent minimally invasive rotary excision of breast benign tumors in the First Affiliated Hospital of Henan University of Traditional Chinese Medicine were selected.According to different positioning methods,they were divided into the"four points and one line"positioning method group(55 cases)and in the traditional positioning method group(57 cases),the observation group used the"four points and one line"positioning method,and the control group used the traditional positioning method.The operations of the two groups of patients were performed by the same professional and skilled surgical team.The number of times,intraoperative blood loss,operation time,postoperative pain score,and occurrence of hematoma.Results The number of needle adjustments in the observation group and the control group were(1.31±1.63)times and(4.47±63.03)times,the blood loss during the operation was(14.82±14.01)mL and(38.25±19.74)mL,and the operation time was(33.58±22.73)minutes and(44.65±5).After 22.47 minutes,the postoperative pain scores were(1.02±1.05)points and(2.54±1.00)points respectively,and the hematoma diameters were(1.36±5.48)mm and(24.65±19.60)mm respectively,and the P values were all less than 0.05.The intraoperative blood loss,operation time,postoperative pain score,and occurrence of hematoma were significantly lower than those in the control group(P<0.05).Conclusion"four points and one line"positioning method can significantly improve the positioning accuracy,shorten the operation time,and reduce the risk of surgery.Moderate blood loss,reduce postoperative pain,reduce the incidence of postoperative hematoma,and have high safety and practicability.
