耳内镜辅助治疗外耳道真菌病患者的疗效分析

Efficacy of otoendoscopy in treatment of external auditory canal mycosis

目的 分析耳内镜辅助治疗外耳道真菌病患者的疗效。方法 选择2020年9月—2022年10月在郑州大学第一附属医院收治的100例外耳道真菌病患者,其中以2020年9月—2021年5月收治患者作为对照组,2021年6月—2022年10月收治患者作为研究组。对照组(50例)行常规外耳道清理联合曲安奈德益康唑乳膏治疗,研究组(50例)采用耳内镜下曲安奈德益康唑乳膏外耳道涂布治疗,评估两组的临床疗效,记录两组的症状改善时间及不良反应发生情况,检测两组的炎症反应指标。结果 研究组的总有效率(92.00%)明显高于对照组(76.00%)(P<0.05),不良反应总发生率(10.00%)明显低于对照组(32.00%)(P<0.05)研究组的耳痒、阻塞感、耳痛、听力下降等症状改善时间明显低于对照组(P<0.05)。研究组的血清TNF-α、IL-6、CRP水平均低于对照组(P<0.05)。结论 耳内镜辅助曲安奈德益康唑乳膏可有效提高外耳道真菌病患者的疗效,还能有效促进症状改善,缓解炎症反应,具有副作用小的优点,值得临床应用。

Objective To analyze the efficacy of otoendoscopy in the treatment of external auditory canal mycosis.Methods A total of 100 patients with external auditory canal fungal disease admitted to the First Affiliated Hospital of Zhengzhou University from September 2020 to October 2022 were selected.Among them,patients admitted from September 2020 to May 2021 were set as the control group,and patients admitted from June 2021 to October 2022 were set as the study group.The control group(50 cases)was treated with routine external auditory canal cleaning combined with triamcinolone acetonide and econazole cream,and the study group(50 cases)was treated with triamcinolone acetonide and econazole cream under otoscopy.The clinical efficacy of the two groups was evaluated,the symptom improvement time and adverse reactions of the two groups were recorded,and the inflammatory response indicators of the two groups were detected.Results The total effective rate of the study group(92.00%)was significantly higher than that of the control group(76.00%)(P<0.05).The improvement time of ear itching,obstruction,otalgia,hearing loss and other symptoms in the study group was significantly lower than that in the control group(P<0.05).The serum levels of TNF-α,IL-6 and CRP in the study group were lower than those in the control group(P<0.05).The total incidence of adverse reactions in the study group(10.00%)was significantly lower than that in the control group(32.00%)(P<0.05).Conclusion Otoendoscopy combined with triamcinolone acetonide and econazole cream can effectively improve the curative effect of patients with external auditory canal mycosis,effectively promote the improvement of symptoms,relieve inflammatory response,and has the advantages of less side effects,which is worthy of clinical application.
