Effects of Snyder’s Hope Theory Model Nursing on PICC Catheterization

目的分析Snyder希望理论模型护理对PICC置管患者拔管情况及生活质量的影响。方法回顾性分析2021年3月—2022年5月泉州市第一医院收治的100例PICC置管患者的临床资料,根据入院时间的先后顺序对患者进行分组,先期入院并采用常规护理的50例患者设为参考组,后期入院采用Snyder希望理论模型护理的50例患者设为研究组,对比两组PICC置管患者拔管情况(计划性拔管、非计划性拔管、意外拔管)、导管相关并发症发生率(指标包括导管滑脱、导管回缩、静脉炎、穿刺点感染)以及干预前后生活质量评分改善情况。结果研究组PICC置管患者计划性拔管、非计划性拔管、意外拔管等总拔管率显著低于参考组(P<0.05);且研究组导管滑脱、导管回缩、静脉炎、穿刺点感染等导管相关并发症总发生率显著低于参考组(P<0.05);两组PICC置管患者干预前各项生活质量评分比较,差异无统计学意义(P>0.05),干预后两组PICC置管患者各项生活质量评分均得到很大的提升,研究组PICC置管患者各项生活质量评分与参考组比较差异有统计学意义(P<0.05)。结论Snyder希望理论模型护理能有效降低PICC置管患者拔管率和导管相关并发症,提高患者的生活质量。

Objective To analyze the impact of Snyder’s Hope Theory model nursing on the extubation status and quality of life of PICC catheterization patients.Methods A retrospective analysis was conducted on the clinical data of 100 PICC catheterization patients admitted to Quanzhou First Hospital from March 2021 to May 2022.Patients were divided into groups based on the order of admission time.50 patients who were initially admitted and received routine care were set as the reference group,while 50 patients who were later admitted and received Snyder Hope theory model nursing care were set as the research group.Compare the extubation status(planned extubation,unplanned extubation,accidental extubation),incidence of catheter related complications(indicators including catheter slip,catheter retraction,phlebitis,puncture site infection),and improvement in quality of life scores before and after intervention between two groups of PICC catheterization patients.Results The total extubation rates of planned extubation,unplanned extubation and accidental extubation in the study group were significantly lower than those in the reference group(P<0.05).The total incidence of catheter related complications such as catheter slip,catheter retraction,phlebitis and puncture site infection in the study group was significantly lower than that in the reference group(P<0.05).Before intervention,there was no statistically significant difference in the quality of life scores of PICC catheterization patients between the two groups(P>0.05).After intervention,the quality of life scores of PICC catheterization patients in the two groups were greatly improved.Compared with the reference group,there was statistically significant difference in the quality of life scores of PICC catheterization patients in the study group and the reference group(P<0.05).Conclusion Snyder hopes that theoretical model nursing can effectively reduce the extubation rate and catheterrelated complications in patients with PICC catheterization,and improve the quality of life of patients.
