实施等级医院评审对临床输血管理持续改进的促进作用研究

Study on the Promoting Effect of Implementing Grade Hospital Evaluation on Continuous Improvement of Clinical Blood Transfusion Management

目的研究等级医院评审对临床输血管理持续改进的促进作用。方法选择吉林省一汽总医院实施等级医院评审前(2019年7月—2020年12月)接受输血治疗的患者360例设为对照组,选择吉林省一汽总医院实施等级医院评审后(2021年7月—2022年12月)接受输血治疗的患者360例设为研究组,统计比较两组患者输血等待时间、病历记录缺陷发生情况及病历书写质量。结果与对照组比较,研究组输血等待时间更短,比较差异有统计学意义(P<0.05)。在病历记录缺陷方面,除血浆适应证不符外,研究组其他病历记录缺陷的发生率均明显低于对照组,比较差异均有统计学意义(P<0.05)。在病历书写质量方面,研究组零缺陷率明显高于对照组,中度缺陷率、重度缺陷率均明显低于对照组,比较差异均有统计学意义(P<0.05);针对轻度缺陷率而言,两组病历比较差异无统计学意义(P>0.05)。结论临床输血管理持续改进中实施等级医院评审后,能够显著缩短患者输血等待时间,减少病历记录缺陷情况的发生,提高病历书写质量。

Objective To study the promoting effect of grade hospital evaluation on continuous improvement of clinical blood transfusion management.Methods A total of 360 patients who received blood transfusion treatment before the implementation of grade hospital evaluation in Jilin Province FAW General Hospital(July 2019 to December 2020)were selected as the control group,and 360 patients who received blood transfusion treatment after the implementation of grade hospital evaluation in Jilin Province FAW General Hospital(July 2021 to December 2022)were selected as the research group.The waiting time for blood transfusion of patients,the occurrence of medical record defects,and the writing quality of the two groups were observed and compared statistically.Results Compared with the control group,the waiting time for blood transfusion in the study group was shorter,the difference was statistically significant(P<0.05).In terms of medical record defects,except for inconsistent plasma indications,the incidence of other record defects in the study group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).In terms of writing quality of medical records,the zero defect rate in the study group was significantly higher than that in the control group,while the moderate defect rate and severe defect rate were significantly lower than those in the control group,the differences were statistically significant(P<0.05).For the rate of mild defects,there was no statistical difference between the two groups(P>0.05).Conclusion The implementation of grade hospital evaluation in the continuous improvement of clinical blood transfusion management can significantly shorten the waiting time for patients to receive blood transfusion,reduce the occurrence of defects in medical records,and improve the quality of medical records.
