PTCD联合茵陈利胆汤治疗胆管癌合并梗阻性黄疸的临床效果

Clinical Effect of Percuteneous Transhepatic Cholangial Drainage Combined With Yinchen Lidan Decoction on the Treatment of Cholangiocarcinoma Combined With Obstructive Jaundice

目的对经皮肝胆管穿刺引流术(percuteneous transhepatic cholangial drainage,PTCD)联合茵陈利胆汤应用于胆管癌合并梗阻性黄疸患者的临床效果进行评价。方法研究对象为2020年6月—2022年6月福建医科大学附属第一医院收治的胆管癌合并梗阻性黄疸患者,共计入组80例。根据治疗方案的不同将其分成对照组和治疗组。对照组中40例采用PTCD进行治疗,治疗组中40例患者采用PTCD联合茵陈利胆汤方案进行治疗。对比两组相关评价指标、不良反应和总有效率。结果治疗后,两组谷草转氨酶、谷丙转氨酶、黄疸指数、血清总胆红素水平均低于治疗前,且治疗组低于对照组(P<0.05)。治疗组疼痛消失时间、黄疸症状消失时间、住院总时间短于对照组(P<0.05)。不良反应发生率比较,对照组高于治疗组(P<0.05)。治疗组总有效率高于对照组(P<0.05)。结论胆管癌合并梗阻性黄疸患者采用PTCD联合中药茵陈利胆汤方案进行治疗的临床效果明显,值得关注。

Objective To evaluate the clinical effect of percutaneous transhepatic cholangial drainage(PTCD)combined with Yinchen Lidan decoction in the treatment of cholangiocarcinoma with obstructive jaundice.Methods The study subjects were selected from June 2020 to June 2022 in the First Affiliated Hospital of Fujian Medical University.There were 80 patients with Cholangiocarcinoma complicated with obstructive jaundice in the group.They were divided into control group and treatment group according to different treatment plans.Forty cases in the control group were treated with PTCD,forty patients were treated with PTCD combined with Yinchen Lidan decoction.The relevant evaluation indicators,adverse reactions and total effective rate between the two groups were compared.Results After treatment,the levels of Aspartic acid Transaminase,alanine Transaminase,jaundice index and serum total bilirubin in the treatment group and the control group were lower than those before treatment,and the treatment group was lower than the control group(P<0.05).The pain relief time,jaundice symptom relief time,and total hospitalization time in the treatment group were shorter than those in the control group(P<0.05).The incidence of adverse reactions in the control group was higher than that in the treatment group(P<0.05).The total effective rate of the treatment group was higher than that of the control group(P<0.05).Conclusion The clinical effect of PTCD combined with Yinchen Lidan Decoction in the treatment of cholangiocarcinoma with obstructive jaundice is obvious and deserves attention.
