血清胃蛋白酶原、胃泌素-17、幽门螺杆菌在上消化道溃疡合并出血中的风险预测 被引量:1

Risk prediction of serum pepsinogen,gastrin-17,and Helicobacter pylori in upper gastrointestinal ulcers with bleeding

摘要 目的探讨血清胃蛋白酶原(PG)、胃泌素-17(G-17)、幽门螺杆菌(Hp)在上消化道溃疡患者中合并出血的风险预测。方法纳入本院2019年1月—2022年6月因上消化道溃疡收治入院的203例患者,按照胃镜检查结果分为出血组和非出血组,分析2组一般资料、Hp阳性率、PG、G-17水平,利用统计软件进行相关分析。结果出血组男性比例、G-17水平较非出血组明显升高,2组比较有统计学意义(P<0.05)。logistic回归模型分析发现,男性、G-17是上消化道溃疡合并出血的独立预测因子。男性、G-17诊断上消化道溃疡合并出血的曲线下面积(AUC)分别为0.594、0.654;其敏感度分别为0.803、0.763,特异度分别为0.386、0.551,G-17最佳截断值为5.16 pmol/L。结论G-17在上消化道溃疡合并出血的患者中具有较高的敏感度和特异度,对上消化道溃疡是否合并出血具有一定的筛查能力和诊断能力,值得临床推广。 Objective This paper aims to investigate the risk prediction of serum pepsinogen(PG),gastrin-17(G-17),and Helicobacter pylori(Hp)in patients with upper gastrointestinal ulcers.Methods A total of 203 patients admitted to our hospital for upper gastrointestinal ulcer from January 2019 to June 2022 were included and divided into bleeding group and non-bleeding group according to the results of gastroscopy,and the general data,the positive rate of Hp,the PG and G-17 levels of the two groups were analyzed,and relevant analysis was carried out by statistical software.Results The proportion of male cases and the level of G-17 in the bleeding group were higher than those in the non-bleeding group,and the differences were statistically significant(P<0.05).Logistic regression model analysis found that male and G-17 were independent pre⁃dictors for upper gastrointestinal ulcer and bleeding.The area under the curve(AUC)of upper gastrointestinal ulcer complicat⁃ed with bleeding in males and G-17 was respectively 0.594 and 0.654,the sensitivity was 0.803 and 0.763,the specificity was 0.386 and 0.551,and the optimal cut-off value of G-17 was 5.16 pmol/L.Conclusion G-17 has high sensitivity and specificity in patients with upper gastrointestinal ulcer and bleeding,and has certain screening ability and diagnostic ability for upper gastrointestinal ulcer combined with bleeding,which is worthy of clinical promotion.
