摘要
目的探讨丹参川芎嗪联合尤瑞克林治疗对老年急性脑梗死(ACI)患者中枢神经特异性蛋白(S100β)、脂蛋白相关磷脂酶A2(Lp-PLA2)水平的影响。方法选择86例老年ACI患者,根据随机数字表法分为对照组43例,患者予西医基础治疗联合尤瑞克林治疗,观察组43例在对照组基础上联合丹参川芎嗪注射液治疗,连续治疗2周。比较2组治疗前后神经功能缺损评分(NIHSS)、Barthel指数评定量表(BI)、血液流变学指标、中枢神经特异性蛋白(S100β)、脂蛋白相关磷脂酶A2(Lp-PLA2)水平变化,并统计2组治疗的总有效率和不良反应发生情况。结果治疗前2组NIHSS评分、Barthel指数评分、各项血液流变学指标、S100β、Lp-PLA2水平比较差异无统计学意义(P>0.05);治疗后2组NIHSS评分、Barthel指数评分、血液流变学指标、S100β、Lp-PLA2水平比较差异有统计学意义(P<0.05);观察组总有效率(90.70%)明显高于对照组(69.77%)(χ^(2)=5.939,P<0.05);2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论丹参川芎嗪注射液联合尤瑞克林治疗老年ACI疗效显著,能够调节脑血流动力学,减轻神经功能缺损,作用机制可能与抑制S100β、Lp-PLA2表达有关。
Objective This paper aims to investigate the effect of Salvia miltiorrhiza and ligustrazine combined with urinary kallidinogenase on the levels of central nervous system specific protein(S100β)and lipoprotein associated phospholipase A2(Lp-PLA2)in elderly patients with acute cerebral infarction(ACI).Methods Eighty six elderly patients with ACI were selected and divided into the control group(43 cases)and the observation group(43 cases)according to the random number table method.The control group received basic treatment of Western medicine combined with urinary kallidinogenase,and the observation group received the treatment with Salvia Miltiorrhiza and Ligustrazine Injection on the basis of the treatment of the control group.The course of treatment was 2 weeks.The neurological deficit score(NIHSS),Barthel Index Scale(BI),hemo⁃rheological indexes,central nervous system specific protein(S100β),and lipoprotein related phospholipase A2(Lp-PLA2)levels were compared between the two groups before and after treatment,and the total effective rate and adverse reactions of the two groups were counted.Results There was no statistical significance on the difference in the NIHSS score,Barthel index score,hemorheological indexes,S100β,and Lp-PLA2 level(P>0.05).The differences of NIHSS score,Barthel index score,hemorheological indexes,S100β,and Lp-PLA2 level after treatment was statistically significant(P<0.05).The total effective rate of the observation group(90.70%)was higher than that of the control group(69.77%)(χ^(2)=5.939,P<0.05).There was no statistical significance on the difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Salvia Miltiorrhiza and Ligustrazine Injection combined with urinary kallidinogenase are effective in the treatment of ACI in the elderly,which can effectively regulate cerebral hemodynamics and alleviate neurological deficits.The mechanism may be related to the inhibition of S100βand Lp-PLA2 expression.
