糖化血红蛋白在献血者筛查中的价值

摘要 目的了解献血者中HbA_(1c)异常的情况,为制定针对性的策略提供依据。方法对本站2018年7月—2022年6月所有符合捐献单采血小板献血条件者增加HbA_(1c)指标检测,测量值超过6.0%视为异常,暂缓献血并建议到医院进一步检查。3个月内该指标在正常范围的,免于此项检测。按性别、献血次数、户籍、年龄段、文化程度、BMI(体质量指数)等不同维度进行统计分析。结果4年间共捐献血小板8531人次,1988人参加了血小板捐献,发现HbA_(1c)异常人数为58人。在不同年龄段和不同BMI间差异有统计学意义。46岁~55岁年龄组比率最高,为4.37%(15/343),36岁以上年龄组所占比68.97%(40/58);BMI 24.0~27.9组别的异常率最高,为4.02%(45/1119),BMI大于24.0献血者占比为79.31%(46/58)。结论HbA_(1c)异常与年龄段、BMI等密切相关。随着年龄增长和BMI增高,HbA_(1c)异常比例成明显增高趋势。有必要对36岁以上和/或BMI大于24.0的献血者进行HbA_(1c)筛查。 Objective This paper aims to understand the abnormal situation of HbA_(1c) in blood donors,so as to provide basis for formulating targeted strategies.Methods HbA_(1c) index test was added to all who met the conditions for blood donation from July 2018 to June 2022.If the measured value exceeded 6.0%,it was regarded as abnormal.Blood donation was postponed and fur⁃ther inspection was recommended to the hospital.If the index is within the normal range within 3 months,the test is exempted.Statistical analysis was conducted according to different dimensions such as gender,blood donation times,registered residence,age group,education level,BMI(height and body mass index).Results During the four years,8531 platelet donations were observed,1988 people participated in platelet donation,and 58 people were found with abnormal HbA_(1c).There are differences between different age groups and different BMIs.The highest rate was 4.37%(15/343)in the age group of 46-55 years old,and 68.97%(40/58)in the age group over 36 years old;the highest abnormal rate was 4.02%(45/1119)in the group of BMI 24.0-27.9,and 79.31%(46/58)of blood donors had a BMI greater than 24.0.Conclusion Abnormal HbA_(1c) is closely related to age and BMI.With the increase of age and BMI,the proportion of abnormal HbA_(1c) increased.It is necessary to screen HbA_(1c) for blood donors over 36 years of age and/or with a BMI greater than 24.0.
