二甲双胍+门冬胰岛素治疗妊娠糖尿病患者的效果及对母婴结局的影响

Effect of Metformin+Insulin Aspart in the treatment of patients with gestational diabetes mellitus and its influence on maternal and infant outcomes

目的浅析二甲双胍+门冬胰岛素治疗妊娠糖尿病患者的效果及对母婴结局的影响。方法选取2020年1月至2022年11月北京市房山区妇幼保健院内科收治的200例妊娠糖尿病患者,经抽签法分为两组,每组各100例。参照组单独使用门冬胰岛素治疗,试验组同时服用二甲双胍,对比两组治疗效果。结果试验组治疗后的糖化血红蛋白、餐后2 h血糖、空腹血糖水平低于参照组,差异有统计学意义(P<0.05)。试验组新生儿呼吸窘迫、低血糖、新生儿黄疸、巨大儿发生率均低于参照组,差异有统计学意义(P<0.05)。试验组胎膜早破、早产、剖宫产率均低于参照组,差异有统计学意义(P<0.05)。结论二甲双胍+门冬胰岛素可以提高妊娠糖尿病患者血糖控制效果,优化新生儿结局和妊娠结局,值得推广应用。

Objective To analyze the effect of Metformin+Insulin Aspart in the treatment of patients with gestational diabetes mellitus and its influence on maternal and infant outcomes.Methods A total of 200 patients with gestational diabetes mellitus were selected admitted to the Internal Medicine Department of Fangshan District Maternal and Child Health Hospital in Beijing from January 2020 to November 2022,and they were divided into two groups by the lot drawing method,with 100 cases in each group.The reference group was treated with Insulin Aspart alone,and the experimental group was given Metformin at the same time,and the treatment effects of the two groups were compared.Results The levels of glycosylated hemoglobin,2 h postprandial blood glucose,and fasting blood glucose in the experimental group after treatment were lower than those in the reference group,with statistical significance(P<0.05).The incidence of neonatal respiratory distress,hypoglycemia,neonatal jaundice and macrosomia in the experimental group was lower than that in the reference group,with statistical significance(P<0.05).The rates of premature rupture of membranes,premature delivery,and cesarean section in the experimental group were lower than those in the reference group,with statistical significance(P<0.05).Conclusion Metformin+Insulin Aspart can improve the effect of blood glucose control in patients with gestational diabetes mellitus,and can also optimize the neonatal outcomes and pregnancy outcomes,which is worthy of promotion and application.
