飞行人员甲状腺抗体检查必要性的初步探讨

Thyroid antibody tests are critical to aircrews

目的 调查分析飞行人员甲状腺抗体检查结果的总体情况及不同年龄、不同飞行机种之间阳性率的差异。方法 采用回顾性研究,选取2020年7月-2021年11月联勤保障部队大连康复疗养中心疗养的105名空勤飞行人员为研究对象,按照不同年龄及飞行机种将其分组,选取《健康体检人群甲状腺抗体与甲状腺功能的检验效果分析》文章中的10 0 0名男性健康体检者为对照组。采取空腹静脉血检测甲状腺抗体水平,比较不同年龄及飞行机种飞行人员组间的抗体阳性率差异。结果 105名飞行人员中抗甲状腺过氧化物酶抗体(anti-thyroidperoxidaseantibody,TPOAb)阳性率为11.43%,抗甲状腺球蛋白抗体(anti-thyroglobulinantibody,TGAb)阳性率为19.05%,均高于健康体检对照组,且差异均有统计学意义(χ^(2)=10.210、41.901,P=0.0 01、<0.0 01);不同年龄组及不同飞行机种组间飞行人员T POAb及TGAb阳性检出率差异均无统计学意义(χ^(2)=1.480、2.994、0.190、0.680,P=0.220、0.084、0.980、0.890)。结论 飞行人员与一般健康体检者相比,甲状腺抗体阳性发生率较高,建议将各年龄段的飞行人员甲状腺抗体的化验检查由医学体检鉴定的选查项目纳入其必查项目中,以便对飞行人员甲状腺相关疾病早发现、早诊断、早治疗。

Objective To study the results of examination of thyroid antibodies among aircrews and the difference in the positive rate between aircrews of different age groups and for different types of aircraft.Methods The clinical data of a total of 105 aircrews who recuperated in Dalian Rehabilitation Center of PLA Joint Logistic Support Force between July 2020 and November 2021 was retrospectively analyzed.All the subjects were grouped by age and aircraft types.Another 1000 healthy male subjects selected in the article Analysis of the Test Effect of Thyroid Antibody and Thyroid Function in Healthy Physical Examination Population served as the control group.Fasting venous blood was taken to detect the level of thyroid antibodies,the positive rate was calculated,and the differences in the positive rate of antibodies between aircrews of different groups were studied.Results Among the 105 aircrews,the positive rate of the thyroid anti-peroxidase antibody(TPOAb)was 11.43%,compared with 19.05%for the thyroglobulin antibody(TGAb).The percentage of aircrews positive for these two thyroid antibodies was significantly higher than in the control group(χ^(2)=10.210,41.901,P=0.001,0.001).The difference in the positive rate of TPOAb and TGAb between different age groups and between pilot groups for different types of aircraft was not statistically significant(χ^(2)=1.480,2.994,0.190,0.680,P=0.220,0.084,0.980,0.890).Conclusion The positive rate of thyroid antibodies in aircrews is higher than in healthy populations.It is recommended that the optional thyroid antibody test of aircrews during physical examination be made mandatory so as to detect,diagnose and treat thyroid-related diseases in aircrews early.
