柴芍龙牡汤加减联合红蓝光治疗肝郁血热型玫瑰痤疮的美学效果及皮肤镜下变化分析

Aesthetic Effects and Dermoscopic Changes in the Treatment of Liver Depression and Blood Heat Type Acne Rosacea by Combining Modifi ed Chaishao Longmu Decoction with Red and Blue Light

目的:探讨柴芍龙牡汤加减联合红蓝光治疗肝郁血热型玫瑰痤疮的美学效果及皮肤镜下改变。方法:选取2019年2月-2022年3月在笔者医院接受治疗的120例肝郁血热型玫瑰痤疮患者,按照随机数字表法分成研究组与对照组,各60例。对照组单纯接受红蓝光治疗,研究组在对照组的基础上给予柴芍龙牡汤治疗。治疗4周后,比较两组临床疗效、治疗前后中医证候积分、治疗后美学效果[温哥华瘢痕量表(Vancouver scar scale,VSS)、面部纹理值、面部毛孔值及皮肤镜下特征]。结果:治疗前,两组中医证候积分、VSS评分、面部纹理值、面部毛孔值差异无统计学意义(P>0.05);治疗后,研究组治疗总有效率高于对照组,中医证候积分、VSS评分、面部纹理值、面部毛孔值及总分均低于对照组,差异有统计学意义(P<0.05)。两组皮肤镜下背景颜色、血管排列方式及血管形状差异有统计学意义(P<0.05)。结论:柴芍龙牡汤加减联合红蓝光治疗肝郁血热型玫瑰痤疮具有较好临床疗效,有效恢复面部皮肤美观,可改善皮肤镜下特征,值得临床推广应用。

Objective This study aimed to investigate the cosmetic outcomes of modifi ed Chaishao Longmu Decoction combined with red and blue light therapy in the treatment of acne rosacea associated with liver depression and blood heat,as well as the observed changes under dermatoscopy.Methods A total of 120 patients with acne rosacea associated with liver depression and blood heat type were selected from February 2019 to March 2022.The patients were randomly divided into a study group and a control group,with 60 cases in each group.The control group received treatment with red and blue light therapy alone,while the study group received treatment with modified Chaishao Longmu Decoction combined with red and blue light therapy.After 4 weeks of treatment,the clinical effects,Traditional Chinese Medicine(TCM)symptom scores before and after treatment,and cosmetic outcomes[assessed by Vancouver Scar Scale(VSS),facial texture value,and facial pore value]after treatment were compared between the two groups.Furthermore,the characteristics observed under dermatoscopy were evaluated.Results No significant diff erence in TCM symptom scores,VSS scores,facial texture value,and facial pore value between the two groups was observed before treatment(P>0.05).However,after treatment,the study group showed a higher total treatment response rate compared to the control group.The TCM symptom scores,VSS score,facial texture value,and facial pore value were lower in the study group than in the control group(P<0.05).The background color,arrangement,and shape of blood vessels observed under dermatoscopy were significantly different between the two groups(P<0.05).Conclusion Modified Chaishao Longmu Decoction combined with red and blue light therapy is effective in the treatment of liver-depression-blood-heat type acne rosacea.The combined treatment can eff ectively improve the appearance of facial skin and modify the observed characteristics under dermatoscopy,which is worthy of clinical application.
