血乳酸、Ang-2、IL-6检测对重症社区获得性肺炎病情及预后的评估价值

Evaluation value of blood lactate,Ang-2,and IL-6 tests for the condition and prognosis of severe community-acquired pneumonia

目的探讨血乳酸(LAC)、促血管生成素2(Ang-2)、白细胞介素6(IL-6)水平对重症社区获得性肺炎病情及预后的评估价值。方法选取2020年2月-2022年8月萍乡赣西肿瘤医院收治的重症社区获得性肺炎患者110例,根据急性生理与慢性健康评分(APACHEⅡ)将患者分为重症组(20分以上)和非重症组(小于20分)两组,重症组患者53例,非重症组患者57例。比较两组患者LAC、Ang-2、IL-6水平,比较存活组、死亡组患者一般临床资料,分析LAC、Ang-2、IL-6水平与APACHE II评分的相关性,采用多因素分析方法分析影响重症社区获得性肺炎患者预后结局的相关因素,绘制受试者工作特征曲线(ROC)分析LAC、Ang-2、IL-6水平预测重症社区获得性肺炎患者预后的评估价值。结果非重症组患者LAC、Ang-2、IL-6均低于重症组,差异有统计学意义(P<0.05),重症社区获得性肺炎患者LAC、Ang-2、IL-6水平与APACHE II评分存在显著正相关(P<0.05),进行Logistic回归分析,结果显示:氧合指数、LAC、Ang-2、IL-6是重症社区获得性肺炎患者预后情况的影响因素(P<0.05)。结论重症社区获得性肺炎患者LAC、Ang-2、IL-6与预后结局有关,各指标对于预测重症社区获得性肺炎患者预后有较好的应用价值。

Objective To evaluate the value of blood lactate(LAC),proangiogenesis hormone 2(Ang-2),and interleukin 6(IL-6)on the condition and prognosis of severe community-acquired pneumonia.Methods 110 patients with severe community acquired pneumonia admitted to our hospital from February 2020 to August 2022 were selected,and divided into two groups in the severe group(more than 20 points)and the non-severe group(less than 20 points),53 patients in the severe group and 57 patients in the non-severe group.We compared the LAC,Ang-2 and IL-6 in the two groups,compared the general clinical data of patients in the surviving and death groups,analyzed the correlation between LAC,Ang-2 and IL-6 and APACHE II score,analyzed the related factors affecting the outcome of patients with severe community-acquired pneumonia,and drew the working characteristic curve(ROC)to analyze LAC,Ang-2 and IL-6 to predict the prognosis of patients with severe community-acquired pneumonia.Results In the mild group,LAC,Ang-2 and IL-6 were lower than the severe group,statistically significant difference(P<0.05),there were significant positive correlation between LAC,Ang-2,IL-6 levels and APACHE II score in patients with severe community-acquired pneumonia(P<0.05),conduct Logistic regression analysis,and the results show that oxygenation index,LAC,Ang-2,and IL-6 are influencing factors for the prognosis of severe community acquired pneumonia patients(P<0.05).Conclusion LAC,Ang-2,and IL-6 in patients with severe community acquired pneumonia are related to prognostic outcomes,and each index has a good application value for predicting the prognosis of patients with severe community acquired pneumonia.
