摘要
目的探讨术前全身炎性反应指数(systemic inflammatory response index,SIRI)、纤维蛋白原与白蛋白比值(fibrinogen to albmin ratio,FAR)联合评分(SIRI-FAR)与食管鳞状细胞癌(esophageal squamous cell carcinoma,ESCC)患者预后的相关性。方法回顾性选取2015年1月~2018年12月于秦皇岛市第一医院行手术治疗的139例ESCC患者为研究对象,收集患者的一般资料、临床病理资料,确定SIRI-FAR最佳截断值并进行分组,比较各组SIRI-FAR与临床病理指标的相关性,应用多因素Logistic回归模型及Nomogram预测模型探讨影响患者3年生存率的因素。结果SIRI、FAR最佳截断值分别为1.39和0.09。根据最佳截断值分为3组,组间比较显示,SIRI-FAR评分与病灶最大径、TNM分期、淋巴结转移及组织分化程度有关(P<0.05)。多因素Logistic回归分析结果显示,淋巴结转移、术后辅助治疗、分化程度及SIRI-FAR 2分组是影响ESCC患者预后的独立危险因素。结论术前SIRI-FAR评分在预测ESCC患者预后中起积极作用,术前SIRI-FAR评分越高,预后越不佳。
Objective To investigate the relationship between preoperative systemic inflammatory response index(SIRI)and fibrinogen to albmin ratio(FAR)combined score(SIRI-FAR)and esophageal squamous cell carcinoma(ESCC)patient prognosis.Methods The 139 ESCC patients who underwent surgery in Qinhuangdao First Hospital from January 2015 to December 2018 were retrospectively selected as the research objects,the general data and clinicopathological data of the patients were collected,the optimal cutoff value of SIRI-FAR was determined and grouped.The correlation between SIRI-FAR and clinicopathologic indexes of each group was compared,and multivariate Logistic regression model and Nomogram prediction model were used to explore the factors affecting the 3-year survival rate of patients.Results The best cut-off values of SIRI and FAR were 1.39 and 0.09,respectively.The patients were divided into 3groups according to the best cut-off value.The comparison between groups showed that the SIRI-FAR score was related to the maximum tumor diameter,TNM stage,lymph node metastasis and tissue differentiation degree(P<0.05).Multivariate Logistic regression analysis showed that lymph node metastasis,postoperative adjuvant therapy,degree of differentiation and SIRI-FAR 2 score group were independent risk factors for the prognosis of patients with postoperative ESCC.Conclusion The preoperative SIRI-FAR score plays an active role in predicting the prognosis of patients with ESCC after operation.The higher the preoperative SIRI-FAR score,the worse the prognosis.
