腹腔镜下胃腔内肿瘤切除术治疗胃不利部位内生型间质瘤的临床分析

Clinical analysis of laparoscopic intragastric surgery for endophytic gastric stromal tumors at unfavorable position

目的评估腹腔镜下胃腔内肿瘤切除术治疗胃不利部位内生型间质瘤的临床疗效与可行性。方法收集2020年4月至2021年11月宁波大学附属第一医院收治的22例行腹腔镜下胃腔内肿瘤切除术的胃不利部位内生型间质瘤患者,回顾性分析患者围术期恢复情况、病理特征和预后。结果22例患者均顺利完成腹腔镜手术,无术中中转开腹。标本假包膜完整,切缘阴性,病理均证实为胃间质瘤。术中出血量(18±8)mL,手术时长(63±12)min,术后肛门排气时间为(20±10)h,术后恢复流质饮食时间(2.4±1.3)d,术后住院时长(6.0±2.0)d。术后患者均顺利出院,未见明显短期并发症。所有患者未见肿瘤局部复发、远处转移。结论腹腔镜下胃腔内肿瘤切除术治疗胃不利部位内生型间质瘤是安全、可行的。

Objective To evaluate the feasibility and clinical efficacy of laparoscopic intragastric surgery for endophytic gastric stromal tumors at unfavorable position.Methods The clinical data of 22 patients who underwent laparoscopic intragastric surgery for endophytic gastric stromal tumors in the First Affiliated Hospital of Ningbo University from April 2020 to November 2021 were colleted.The perioperative recovery,clinicopathologic characteristics of tumors,and follow up results were analyzed retrospectively.Results All patients successfully completed the operation without conversing to open surgery,the intraoperation blood loss was(18±8)mL,and the operation time was(63±12)min,postoperative anal exhaust time was(20±10)h,postoperative liquid diet time was(2.4±1.3)d,and the length of postoperative hospital stay was(6.0±2.0)d.The pathological examination showed gastric stromal tumors with complete pseudocapsule and negative margin.All patients recovered and were discharged without complications.No local recurrence and distant metastasis were observed during the follow-up in all patients.Conclusion Laparoscopic intragastric surgery is a safe and feasible procedure for treating endophytic gastric stromal tumors at unfavorable position.
