长沙市某医院1141例孕妇维生素D水平调查分析

Investigation and analysis of vitamin D levels of 1,141 pregnant women in a hospital in Changsha

目的了解长沙市某医院孕妇维生素D水平,为该地区孕妇人群合理补充维生素D提供参考。方法选取2021年12月至2022年5月在长沙市某三级综合医院产科门诊产检的孕妇1141例,采集受试者清晨空腹静脉血,采用化学发光法检测外周血25(OH)D浓度。将数据分别按照孕期和年龄进行分组,分析不同孕期和不同年龄的孕妇维生素D水平及等级分布情况。结果本组孕妇的维生素D总体水平为51.98(36.08~70.18)nmol/L。孕中、晚期维生素D水平均高于孕早期(P<0.05),维生素D等级分布随孕期增长呈现增高趋势。不同年龄孕妇的维生素D水平及等级分布比较,差异有统计学意义(P<0.05),≤25岁组孕妇维生素D水平低于其他年龄组(P<0.05),维生素D缺乏率高于其他年龄组(P<0.05)。不同年龄组孕早、中、晚期维生素D水平比较中,仅孕早期差异有统计学意义(P<0.05)。结论该院孕妇孕早期维生素D水平偏低,进行维生素D水平的早期筛选和干预对于改善维生素D缺乏现状有重要意义,应重点关注≤25岁的孕妇群体。

Objective To assess the vitamin D levels in pregnant women in a hospital from Changsha,provide reference for vitamin D supplementation for pregnant women.Methods A total of 1,141 pregnant women who underwent prenatal examinations in the obstetric clinic of a tertiary general hospital in Changsha,Hunan Province,China from December 2021 to May 2022 were selected.Fasting venous blood was collected from the subjects in the morning,and the peripheral blood levels of 25-hydroxyvitamin D(25[OH]D)were detected by chemiluminescence method.The data was grouped according to pregnancy and age,and the vitamin D level and distribution of vitamin D grade of pregnant women in different pregnancy and ages were analyzed.Results The mean vitamin D level of pregnant women in a hospital from Changsha was 51.98(36.08-70.18)nmol/L.Vitamin D level in the middle and late trimester of pregnancy was significantly higher than that in the early trimester(P<0.05),and the distribution of vitamin D levels increased with the prolongation of pregnancy.There were significant differences in vitamin D level and distribution of vitamin D grade in pregnant women at different age groups(P<0.05).The vitamin D level of pregnant women younger than 25 years was significantly lower than that in other age groups(P<0.05),and the vitamin D deficiency rate was significantly higher(P<0.05).Vitamin D levels were compared in pregnant women of different age groups during pregnancy,and a significant difference was only detected in the first trimester(P<0.05).Conclusion The vitamin D level of pregnant women in a hospital from Changsha is low in the first trimester.Early screening and intervention of vitamin D levels are of great significance to improve the current situation of vitamin D deficiency.A special attention should be paid to pregnant women younger than 25 years.
