补中益气汤加减联合隔姜铺灸治疗脾胃虚弱型胃溃疡患者的效果

Effect of Buzhong Yiqi Decoction Modified Combined with Ginger Separated Moxibustion in the Treatment of Gastric Ulcer Patients with Spleen and Stomach Weakness Type

目的:探讨补中益气汤加减联合隔姜铺灸治疗脾胃虚弱型胃溃疡患者的效果。方法:选取2020年3月—2022年7月诏安县医院收治的100例脾胃虚弱型胃溃疡患者为研究对象。根据随机数表法将其分为对照组和观察组,各50例。对照组给予常规西药治疗,观察组在对照组基础上采用补中益气汤加减联合隔姜铺灸治疗。比较两组临床疗效,治疗前后中医症候积分、炎症因子、胃肠激素。结果:观察组治疗总有效率为98.00%,高于对照组的82.00%,差异有统计学意义(P<0.05)。治疗后,两组胃胀、胃部隐痛、恶心呕吐、反酸嗳气积分均较治疗前有明显下降,并且观察组胃胀、胃部隐痛、恶心呕吐、反酸嗳气积分均明显低于对照组,差异有统计学意义(P<0.05)。治疗后,两组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均较治疗前明显降低,并且观察组IL-6、TNF-α水平均明显低于对照组,差异有统计学意义(P<0.05)。治疗后,两组胃泌素(GAS)水平较治疗前降低、胃动素(MTL)水平较治疗前升高,并且观察组GAS水平低于对照组,MTL水平明显高于对照组,差异有统计学意义(P<0.05)。结论:补中益气汤加减联合隔姜铺灸治疗脾胃虚弱型胃溃疡可明显提高临床治疗效果,降低中医症候积分,改善患者炎症因子及胃肠激素水平,有临床借鉴价值。

Objective:To investigate the effect of Buzhong Yiqi Decoction Modified combined with ginger separated moxibustion in the treatment of gastric ulcer patients with spleen and stomach weakness type.Method:A total of 100 gastric ulcer patients with spleen and stomach weakness type admitted to Zhao’an County Hospital from March 2020 to July 2022 were selected as study objects.They were divided into control group and observation group according to random number table method,with 50 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Buzhong Yiqi Decoction Modified combined with ginger separated moxibustion on the basis of control group.The clinical efficacy,traditional Chinese medical syndrome score,inflammatory factors and gastrointestinal hormone before and after treatment were compared between the two groups.Result:The treatment total effective rate of the observation group was 98.00%,which was higher than 82.00%of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of stomach distension,stomach dull pain,nausea and vomiting,and acid reflux and belching were significantly decreased in both groups compared with those before treatment,and the scores of stomach distension,stomach dull pain,nausea and vomiting,and acid reflux and belching in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in both groups were significantly decreased compared with those before treatment,and the levels of IL-6 and TNF-αin observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the gastrin(GAS)level decreased compared with those before treatment and motilin(MTL)level increased in both groups compared with those before treatment,and GAS level in the observation group was lower than that in control group,and MTL level was significantly higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Buzhong Yiqi Decoction Modified combined with ginger separated moxibustion in the treatment of gastric ulcer with spleen and stomach weakness type can obviously improve the clinical therapeutic effect,reduce the traditional Chinese medical symptom score,improve the level of inflammatory factors and gastrointestinal hormones,and have clinical reference value.
