中药沐足联合自血疗法与盐酸特比萘芬乳膏治疗足癣的临床观察

Clinical Observation of Traditional Chinese Medicine Footbath Combined with Autoblood Therapy and Terbinafine Hydrochloride Cream in the Treatment of Tinea Pedis

目的:观察中药沐足联合自血疗法与盐酸特比萘芬乳膏对足癣的临床疗效。方法:将2020年9月—2022年9月就诊于河源市中医院的120例足癣患者按随机数表法分为四组,各30例。A组采用重楼参柏洗剂联合自血疗法+盐酸特比萘芬乳膏,B组采用重楼参柏洗剂+盐酸特比萘芬乳膏,C组采用自血疗法+盐酸特比萘芬乳膏,D组仅用盐酸特比萘芬乳膏,四组均治疗30 d。比较四组治疗前后临床症状评分变化,真菌镜检变化、临床疗效、不良反应及复发情况。结果:四组治疗前及治疗15 d、30 d临床症状评分时间、组间及交互效应比较,差异有统计学意义(P<0.05);与治疗前比较,四组治疗15 d、30 d临床症状评分均显著下降,且A组、B组、C组治疗15 d、30 d临床症状评分均显著低于D组,A组治疗15 d、30 d临床症状评分显著低于B组、C组,差异有统计学意义(P<0.05)。四组真菌学疗效、总有效率比较,差异有统计学意义(P<0.05);A组真菌清除率、总有效率显著高于B组、C组、D组,差异有统计学意义(P<0.05)。四组不良反应发生率及复发率比较,差异无统计学意义(P>0.05)。结论:中药沐足联合自血疗法与盐酸特比萘芬乳膏治疗足癣效果显著,能明显改善临床症状评分,提高真菌清除率,减少复发,安全性高。

Objective:To observe the clinical curative effect of traditional Chinese medicine(TCM)footbath combined with autoblood therapy and Terbinafine Hydrochloride Cream on tinea pedis.Method:A total of 120 patients with tinea pedis treated in Heyuan Hospital of Traditional Chinese Medicine from September 2020 to September 2022 were divided into four groups according to random number table method,with 30 cases in each group.Group A was treated with Chonglou Shenbai Lotion combined with autoblood therapy+Terbinafine Hydrochloride Cream,group B was treated with Chonglou Shenbai Lotion+Terbinafine Hydrochloride Cream,group C was treated with autoblood therapy+Terbinafine Hydrochloride Cream,and group D was treated with Terbinafine Hydrochloride Cream only,All patients were treated for 30 d.The changes in clinical symptom scores before and after treatment,fungal microscopy,clinical curative effect,adverse reactions and recurrence were compared among four groups.Result:There were significant differences in clinical symptom score time,inter-group and interaction effects among four groups before treatment and 15 d and 30 d after treatment(P<0.05).Compared with before treatment,the clinical symptom scores of four groups at 15 d and 30 d of treatment were significantly decreased,and the clinical symptom scores of group A,group B and group C at 15 d and 30 d of treatment were significantly lower than those of group D,the clinical symptom scores of group A at 15 d and 30 d of treatment were significantly lower than those of group B and group C,and the differences were statistically significant(P<0.05).There were statistically significant differences in the mycological efficacy and total effective rate among four groups(P<0.05);the fungal clearance rate and total effective rate of group A were significantly higher than those of group B,group C and group D,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions and recurrence rate among the four groups(P>0.05).Conclusion:TCM footbath combined with autoblood therapy and Terbinafine Hydrochloride Cream is effective in the treatment of tinea pedis,which can obviously improve clinical symptom scores,increase fungal clearance rate and reduce recurrence,with high safety.
