药品集中带量采购政策在内蒙古某三甲中医院实施后的药品使用情况及经济学效果

Use and Economic Effect of Centralized Procurement of Drugs in A Third-grade Hospital of Traditional Chinese Medicine in Inner Mongolia

目的 通过分析某三甲中医院实施药品集中带量采购政策后的药品使用情况,评价国家组织药品集中带量采购的有效性及经济性。方法 通过医院信息系统(HIS)采集2020年内蒙古某三甲中医医院使用量排名靠前的降压、降脂、降糖药品,对中选药品和非中选药品的处方量、使用量、销售金额、价格进行比较,并与历史同期药品用量和销售金额进行比较,分析其经济学效果。结果 2020年该院中选药品处方数多于相同通用名下非中选药品处方数,中选药品平均使用量用药频度(DDDs)占比64.77%,非中选药品平均使用量DDDs占比为35.23%;中选药品与非中选药品DDDs比约为2∶1;中选药品价格日均费用(DDDc)(采购价的标准单位)平均为0.58元,非中选药品价格DDDc平均为4.18元;中选药品平均销售金额占比23.81%,非中选药品平均销售金额占比76.19%,潜在费用节省率91.19%;2020年与历史同期相比,使用量同比增长18.08%,销售金额同比增长-61.82%;未发现中选药品存在严重不良反应现象。结论 某三甲中医院实施药品集中带量采购政策后,药品费用明显降低,在一定程度上减轻了患者的经济负担,也减轻了国家医保负担,在医药卫生改革的进程中取得了积极的进展和成效。

Objective To analyse the drug usage situation after implementing the policy of centralized quantity procurement of drugs in a tertiary traditional Chinese medicine hospital,and to evaluate the effectiveness and economy of national organized centralized quantity procurement of drugs.Methods We collected the antihypertensive,lipid-lowering and hypoglycemic drugs in Traditional Chinese Medicine Hospital of Inner Mongolia Autonomous Region in 2020 through HIS,compared the prescription amount,consumption amount,sales amount and price of the selected drugs and non selected drugs,and compared with the drug consumption and sales amount in the same period of history to analyze its economic effects.Results In 2020,the number of prescriptions of selected drugs in the hospital was greater than that of non selected drugs under the same general name.The average DDDs of selected drugs accounted for 64.77%,and the average DDDs of non selected drugs accounted for 35.23%;The DDDs ratio of the selected and non selected drugs is about 2∶1;The average DDDc(Standard unit of purchase price) of selected drugs was 0.58 yuan,and the average DDDc of non-selected drugs was 4.18 yuan.The average sales amount of selected drugs accounted for 23.81%,the average sales amount of non selected drugs accounted for 76.19%,and the potential cost saving rate was 91.19%;Compared with the same period in history in 2020,the usage amount increased by 18.08% and the sales amount increased by-61.82%;No serious adverse reactions of selected drugs were found.Conclusion After the implementation of centralized drug procurement organized by the state in a third-grade hospital of traditional Chinese medicine,the drug cost has been greatly reduced,which can reduce the burden of patients' drug cost to a certain extent,and also reduce the burden of national medical insurance,and has achieved positive progress and results.
