胶质瘤患者心理神经症状群与生活质量和炎症指标的关联

Relation of psychoneurological symptom cluster to quality of life and inflammatory indicators in patients with glioma

目的:了解胶质瘤患者心理神经症状群的潜在类别及其与生活质量的关系,探索与症状群发生有关的炎症因素。方法:选取203例胶质瘤患者,使用多维疲劳量表(MFI-20)、匹兹堡睡眠质量指数(PSQI)、汉密顿焦虑量表(HAMA)、汉密顿抑郁量表(HAMD)及欧洲癌症研究与治疗组织的生活质量核心问卷(EORTC QLQ-C30)评估患者心理神经症状及生活质量,同时检测患者血浆炎症因子水平。结果:胶质瘤患者心理神经症状群类别可分为低症状组、低抑郁症状组和高症状组,高症状组患者生活质量得分低于其他2组,可用于预测高症状组的炎症指标为IL-6(OR=54.96)和TNF-α(OR=65.37)表达水平高。结论:胶质瘤患者心理神经症状群有3个潜在类别,不同类别患者生活质量不同,症状群发生与炎症因子有关。

Objective:To identify psychoneurological symptom cluster latent classes and compare the quality of life among different classes,and explore the inflammatory factors related to symptom cluster in patients with glioma.Methods:Totally 203 patients with glioma were selected and the Multidimensional Fatigue Inventory(MFI-20),Pittsburgh Sleep Quality Index(PSQI),Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD)and EORTC QLQ-C30(QLO-30)were used to evaluate psychoneurological symptom and quality of life of patients.Inflammatory cytokines were simultaneously measured.Results:The samples were divided into three classes as low symptoms group,low depressive symptomgroup and high symptoms group.The EORTC QLQ-C30 scores were lower in the high symptoms group than in the other two groups,and patients in the high symptoms group were characterized by significantly increased levels of IL-6(OR=54.96)and TNF-α(OR=65.37).Conclusion:Three classes of psychoneurological symptom cluster which have significant difference in quality of life are identified,and the occurrence of symptom cluster is related to inflammatory cytokines.
