帕累托图联合鱼骨图分析法用于某院门诊药房调剂差错分析

Application of Pareto Chart Combined with Fishbone Chart Analysis in the Analysis of Dispensing Errors in the Outpatient Pharmacy of a Hospital

目的减少医院门诊药房调剂差错,提高门诊药房服务质量。方法收集医院门诊药房2018年1月至12月(整改前)和2021年1月至12月(整改后)的调剂差错数据,采用帕累托图分析法调剂差错类型,采用鱼骨图分析法查找调剂差错原因,制订整改措施,比较整改前后调剂差错率。结果导致门诊药房调剂差错的主要因素为药品数量错误和药品用法用量错误,次要因素为药品品种错误。整改后,门诊药房调剂差错率由0.0141%降至0.0024%(P<0.001)。结论应用帕累托图联合鱼骨图分析法对门诊药房调剂差错类型及差错原因进行分析,并针对性地改进,能显著减少调剂差错,确保患者用药安全。

Objective To reduce dispensing errors in the outpatient pharmacy of a hospital,and to improve the quality of services in the outpatient pharmacy.Methods The data of dispensing errors before rectification(from January to December 2018)and after rectification(January to December 2021)in the outpatient pharmacy of the hospital were collected.The types of dispensing errors were analyzed by the Pareto chart,and the causes of dispensing errors were found by the fishbone chart analysis to formulate rectification measures,and the dispensing error rates before and after rectification were compared.Results The main factors leading to dispensing errors in the outpatient pharmacy were drug quantity errors and drug usage and dosage errors,and the secondary factor was the drug variety error.After rectification,the dispensing error rate in the outpatient pharmacy decreased from 0.0141%to 0.0024%(P<0.001).Conclusion Application of Pareto chart combined with fishbone chart analysis in analyzing the types and causes of dispensing errors in the outpatient pharmacy,and formulating targeted improvements can significantly reduce dispensing errors and ensure patients'medication safety.
