结直肠癌肾上腺转移的影响因素分析

Analysis of clinical factors of adrenal metastasis in colorectal cancer

目的探讨结直肠癌肾上腺转移的影响因素。方法对我院2015年1月至2016年12月收治的850例结直肠癌患者的临床资料进行回顾性分析。结果结直肠癌肾上腺转移与肿瘤原发部位、肿瘤病理类型、T分期、N分期、是否脉管侵犯、是否合并其他器官转移、癌胚抗原(CEA)水平及手术方式有关(P<0.05);与患者年龄、性别、肿瘤分化程度、肿瘤大小、微卫星状态、Ras基因状态、Braf基因状态无关(P>0.05)。多因素分析显示CEA水平高、未行根治性手术是结直肠癌发生肾上腺转移的独立危险因素。结论CEA水平高是发生肾上腺转移的高危因素,行根治性手术治疗可以改善其预后。

Objective To investigate the clinical features of adrenal metastasis from colorectal cancer.Methods The clinical data of 850 patients with colorectal cancer admitted to Shanxi Provincial Cancer Hospital from January 2015 to December 2016 were retrospectively analyzed.Results Primary tumor in rectum,tumor pathological type,T stage,N stage,vascular invasion,whether combined with other organs metastasis,carcinoma embryonic antigen(CEA)level and surgical method were related to adrenal metastasis of colorectal cancer(P<0.05).However,it was not correlated with age,gender,tumor differentiation degree,tumor size,microsatellite status,Ras gene status and Braf gene status(P>0.05).Multivariate analysis showed that high CEA level and no radical surgery were independent risk factors for adrenal metastasis of colorectal cancer.Conclusion Advanced rectal cancer and high CEA level are high-risk factors for adrenal metastasis.Radical surgical treatment can improve the prognosis.
