中药复方延缓椎间盘退变体外实验研究进展 被引量:1

摘要 目的 综述近5年来中药复方延缓椎间盘退变(interverbral disc degeneration, IDD)体外实验的研究进展。方法 查阅CNKI、万方、PubMed(截止于2022年7月)公开发表的相关文献,从细胞选择、培养方式和作用机制三方面进行阐述。结果 中药复方延缓IDD体外实验对象主要集中在人、大鼠和兔的髓核、纤维环和终板细胞上。施加因素多取自临床常用的经方、验方,尤以补益肝肾及活血化瘀类为主。实验均取得了较为理想的结果,中药对IDD影响的机制多是从促进椎间盘细胞增殖与自噬、抑制其凋亡、抑制炎性因子释放与细胞外基质(extracellular matrix, ECM)的降解等方面进行阐释,涉及的信号通路包括Wnt/β-catenin、NF-κB、MAPK、Fas/FasL、mTOR、PI3K/Akt等。结论 体外实验表明中药复方对IDD模型有明确的影响,其主要作用机制通过影响与IDD密切相关的信号通路、细胞增殖、凋亡与自噬、炎性因子释放与ECM的降解等得到阐释,但研究仍存在一定局限性。 Objective To review the research progress of the in vitro experiment of traditional Chinese medicine compound to delay intervertebral disc degeneration(IDD)in the past five years.Methods By reviewing the relevant literature published by CNKI,WANFANG and PubMed(until July 2022),cell selection,culture mode and mechanism of action were elaborated by us in three aspects.Results The in vitro subjects of TCM compound delaying IDD were mainly concentrated on the nucleus pulposus,fibrous rings and end plate cells of humans,rats and rabbits.The applied factors are mostly taken from the commonly used clinical prescriptions,especially tonifying liver and kidney,promoting blood circulation and removing blood stasis.The ideal experimental results have been obtained.The mechanism of the influence of TCM on IDD is mostly explained from the aspects of promoting the proliferation and autophagy of intervertebral disc cells,inhibiting their apoptosis,inhibiting the release of inflammatory factors and the degradation of extracellular matrix(ECM),involving signaling pathways such as Wnt/β-catenin,nuclear transcription factor-κB(NF-κB),mitogen-activated protein kinase(MAPK),Fas/FasL,mammalian target of rapamycin(mTOR),phosphatidylinositol 3-kinase(PI3K)/protein kinase B(Akt),etc.Conclusion In vitro experiments show that TCM compound has a clear impact on the IDD model,and its main mechanism of action is explained by influencing the signaling pathways closely related to IDD,cell proliferation,apoptosis and autophagy,inflammatory factor release and ECM degradation,but the study still has some limitations.
