甲状腺激素及甲状腺自身抗体检测在亚临床甲状腺功能减退疾病中的临床应用

Clinical Application of Thyroid Hormone and Thyroid Autoantibody Tests in Subclinical Hypothyroidism Diseases

目的:分析亚临床甲状腺功能减退症(简称甲减)患者甲状腺激素及甲状腺自身抗体检测的临床意义。方法:选取2020年10月—2022年10月贵阳市妇幼保健院亚临床甲减患者70例作为试验组,同期到院体检的健康人员70例作为对照组。将试验组中促甲状腺激素(TSH)水平在4.2~8.0 mIU/L的患者作为试验1组,TSH水平>8.0 mIU/L作为试验2组。比较对照组与试验组患者甲状腺球蛋白抗体(抗-TG)、抗甲状腺过氧化物酶抗体(抗-TPO)、促甲状腺素受体抗体(抗-TR)、游离甲状腺素(FT4)、TSH及游离三碘甲状腺原氨酸(FT3)水平,分析对照组及试验1组、试验2组抗-TG、抗-TR、抗-TPO阳性表达率。结果:试验组抗-TG、抗-TPO、抗-TR、TSH水平高于对照组,FT4、FT3水平低于对照组,差异有统计学意义(P<0.001);试验1组、试验2组抗-TR、抗-TPO、抗-TG阳性表达率高于对照组,试验1组抗-TR阳性表达率高于试验2组,抗-TPO、抗-TG阳性表达率低于试验2组,差异有统计学意义(P<0.001)。结论:对亚临床甲减患者采取甲状腺激素、甲状腺自身抗体检测的临床应用价值较高,为临床医师诊疗评估疾病提供了有力依据。

Objective:To analyze the clinical significance of thyroid hormone and thyroid autoantibody detection in patients with subclinical hypothyroidism(hypothyroidism for short).Methods:Seventy cases of patients with subclinical hypothyroidism in Guiyang Maternal and Child Health Hospital from October 2020 to October 2022 were selected as the test group,and seventy cases of healthy people who came to the hospital for physical examination during the same period were selected as the control group.Patients with thyrotropin(TSH)levels between 4.2 and 8.0 mIU/L in the test group were taken as test group 1,and TSH levels>8.0 mIU/L were taken as test group 2.Thyroglobulin antibody(anti-TG),anti-thyroid peroxidase antibody(anti-TPO),thyrotropin receptor antibody(anti-TR),free thyroxine(FT4),TSH,and free triiodothyronine(FT3)levels were compared between patients in the control group and test group.The positive expression rates of anti-TG,anti-TR and anti-TPO were analyzed in the control group and test groups 1 and 2.Results:The levels of anti-TG,anti-TPO,anti-TR,and TSH in the test group were higher than those in the control group,and the levels of FT4 and FT3 were lower than those in the control group,and the difference was statistically significant(P<0.001).The positive expression rates of anti-TR,anti-TPO and anti-TG in test group 1 and test group 2 were higher than those in the control group.The positive expression rate of anti-TR in test group 1 was higher than that in test group 2,and the positive expression rate of anti-TPO and anti-TG was lower than that in test group 2,and the difference was statistically significant(P<0.001).Conclusion:The clinical application value of thyroid hormone and thyroid autoantibody testing in patients with subclinical hypothyroidism is high,providing a strong basis for clinicians to diagnose and evaluate the disease.
