多重耐药菌预防控制措施在慢性阻塞性肺炎患者感染预防控制中的应用研究 被引量:1

摘要 目的观察多重耐药菌预防控制措施在慢性阻塞性肺炎患者感染预防控制中的应用研究。方法纳入本院治疗的慢性阻塞性肺炎患者120例作为研究对象,以入院先后顺序分为两组,对照组选取2021年1-6月入院的60例患者进行常规管理,研究组选取2021年7-12月入院的60例患者进行预防控制措施管理,所有患者在治疗过程中均给予痰标本耐药菌监测,比较两组患者多重耐药菌发病率以及耐药菌分布。结果研究组多重耐药菌检出率为5.00%,明显低于对照组18.33%,且具有统计学差异(P<0.05)。对于主要革兰氏阴性菌,氨苄西林、头孢唑林、头孢曲松、头孢替坦等药物具有较高的耐药率,而亚胺培南、阿米卡星、哌拉西林等药物耐药率较低。对于主要革兰氏阳性菌,青霉素、氨苄西林的耐药率均为100.00%,环丙沙星也有较高的耐药性,而替考拉宁、万古霉素、利奈唑胺的耐药率均为0.00%。结论多重耐药菌预防控制措施能够有效地降低慢性阻塞性肺炎患者的多重耐药菌感染。同时临床应该给予针对性用药,以减少多重耐药菌发生率。 Objective To observe and study application of prevention and control measures for multidrug-resistant bacteria in prevention and control of infection of patients with chronic obstructive pneumonia.Methods The paper chose 120 patients with chronic obstructive pneumonia admitted to our hospital as study objects,and divided them into two groups based on admission sequence.60 cases in control group admitted from January to June 2021 were treated with routine management,60 cases in study group admitted from July to December 2021 with prevention and control measures management.All patients were monitored for drug-resistant bacteria in sputum samples during treatment process,incidence and distribution of multidrug resistant bacteria was compared between two groups.Results Detection rate of multidrug-resistant bacteria in study group was 5.00%,significantly lower than 18.33%in control group,there was statistical difference(P<0.05).Ampicillin,cefazolin,ceftriaxone,cefotetan and other drugs had high resistance rates for main gram-negative bacteria,while imipenem,amikacin,piperacillin and other drugs had low resistance rates.For main gram-positive bacteria,drug resistance rate of penicillin and ampicillin was 100.00%,ciprofloxacin also had high drug resistance,while drug resistance rate of teicoplanin,vancomycin and linezolid was 0.00%.Conclusion Prevention and control measures for multidrug-resistant bacteria can reduce infection of multidrug-resistant bacteria of patients with chronic obstructive pneumonia effectively.At the same time,it is necessary to provide targeted medication in clinical practice to reduce incidence of multidrug-resistant bacteria.
