对慢性阻塞性肺疾病伴重症呼吸衰竭患者实施机械通气的治疗效果

Therapeutic Effect of Mechanical Ventilation in Patients with Chronic Obstructive Pulmonary Disease and Severe Respiratory Failure

摘要 目的:探讨对慢性阻塞性肺疾病(COPD)伴重症呼吸衰竭患者实施机械通气治疗的临床效果。方法:选取2021年4月—2022年4月贵州航天医院收治的80例COPD伴重症呼吸衰竭患者作为研究对象,随机分为参照组与研究组,各40例。参照组给予常规治疗,研究组在参照组基础上给予机械通气治疗。比较两组治疗效果、二氧化碳分压(PCO_(2))、氧分压(PO_(2))、心率及呼吸频率。结果:研究组治疗总有效率高于参照组,差异有统计学意义(P=0.003)。治疗前,两组PCO_(2)、PO_(2)水平比较,差异无统计学意义(P>0.05);治疗后,两组PCO_(2)低于治疗前,研究组低于参照组,两组PO_(2)水平高于治疗前,研究组高于参照组,差异有统计学意义(P<0.001)。治疗前,两组心率、呼吸频率比较,差异无统计学意义(P>0.05);治疗后,两组心率、呼吸频率低于治疗前,且研究组低于参照组,差异有统计学意义(P<0.001)。结论:COPD伴重症呼吸衰竭患者在常规治疗基础上给予机械通气治疗,可显著提高治疗效果,有效降低PCO_(2)水平,提高PO_(2)水平,促进患者良好预后。 Objective:To investigate the clinical effect of mechanical ventilation therapy in patients with chronic obstructive pulmonary disease(COPD)and severe respiratory failure.Methods:A total of 80 COPD patients with severe respiratory failure admitted to Guizhou Aerospace Hospital from April 2021 to April 2022 were selected as the study subjects.They were randomly divided into reference group and study group with 40 cases in each group.The reference group was given conventional treatment,and the study group was given mechanical ventilation therapy on the basis of the reference group.The treatment effect,partial pressure of carbon dioxide(PCO_(2)),partial pressure of oxygen(PO_(2)),heart rate and respiratory rate were compared between the two groups.Results:The total effective rate of treatment in the study group was higher than that in the reference group,and the difference was statistically significant(P=0.003).Before treatment,there was no statistically significant difference in the levels of PCO_(2) and PO_(2) between the two groups(P>0.05).After treatment,PCO_(2) in the two groups was lower than that before treatment,the study group was lower than the reference group;PO_(2) level in the two groups was higher than that before treatment,the study group was higher than the control group,with a statistically significant difference(P<0.001).Before treatment,there was no significant difference in heart rate and respiratory rate between the two groups(P>0.05).After treatment,the heart rate and respiratory rate in the two groups were lower than those before treatment,and the study group was lower than the control group,and the difference was statistically significant(P<0.001).Conclusion:Mechanical ventilation therapy on the basis of conventional treatment in patients with COPD and severe respiratory failure can significantly improve the treatment effect,effectively reduce PCO_(2) level,increase PO_(2) level,and promote good prognosis.
